The evening nursing service in Fredensborg municipality is looking for 2 evening nurses who value tasks closely related to the citizen in the citizen's own home, at a care center or residential facilities, in close collaboration with the region, home care, care centers, residential facilities and 1813.

We work with diverse planned and unplanned tasks and operate across the municipality in cooperation with nursing and SOSU colleagues.

Hospital admissions have become significantly shorter than before, which is reflected in the evening tasks. Likewise, an expanded collaboration with the region is expected in connection with the rollout of the health reform.

The position is initially 28 hours, but we are very receptive to your wishes.

Who are we?

The Fredensborg evening nursing team consists of a stable staff group of 6 permanent nurses, as well as a planner/coordinator for home care groups.

The evening nursing service is integrated into home care, which gives us a strong coherence with our partners.

In connection with the health reform, we have 2 nurses who will transfer to the region. Therefore we are seeking 2 nurses, so even if acute tasks transfer to the region, you will in the evening as a nurse have good prerequisites for being integrated into and supporting the local home care groups and care centers, as well as the good citizen pathways at home.

In the evening shift there are 3 nurses working together, covering the entire municipality and each serving the role of responsible or base nurse.

The position as base nurses is currently available. In time you can, if you wish, be trained to become the responsible nurse.

We have a good collaboration with TST (transsectoral mobile team) from Hillerød Hospital, for example with IV treatments or complex pathways, where the hospital has treatment responsibility. This collaboration is continuously expanded and will, per 1/1 2027, be the regional mobile acute team we collaborate with.

We document in Nexus and use tablet and smartphone.

The 4 home care areas in the municipality (Nivå, Kokkedal, Fredensborg, Humlebæk) have implemented “self-managing teams”.

Typical tasks in the evening shift

• Visit and perform planned nursing tasks.
• Palliative care pathways.
• Coordination and handling of acutely arising tasks.
• Carry out assessment visits after referral from external/internal partners as well as after discharge
• Administer IV treatment in collaboration with the region.
• Participate in triage meetings in home care groups.
• Documentation.

We wish for a new colleague who

• Likes to provide nursing to (typically) elderly citizens in the municipality.
• Is engaged, responsible, has good humor, flexible and proactive.
• Experience as a district nurse and preferably from the secondary sector.
• Wants to provide nursing of the highest quality in collaboration with other professional groups.
• Active participation in developing cooperation and professional sparring with home care groups and care centers.
• Is willing to take on the role of responsible after training.
