The evening nursing service in Fredensborg municipality is seeking 2 evening nurses who value tasks close to the citizen in the citizen's own home, at a nursing home or residences, in close collaboration with the region, home care, nursing homes, residences and 1813.

We work with a variety of planned and unplanned tasks and operate across the municipality in collaboration with nursing and SOSU colleagues.

Hospital admissions have become substantially shorter than before, which is reflected in the evening tasks. Likewise, an expanded collaboration with the region is expected in connection with the rollout of the health reform.

The position is initially 28 hours, but we are certainly receptive to your wishes.

Who are we?

The Fredensborg evening nursing team consists of a stable group of 6 permanently employed nurses, as well as a planner/ coordinator for the home care groups.

The evening nursing service is integrated into the home care, which gives us a strong cohesion with our partners.

In connection with the health reform we have 2 nurses who will transition to the region. Therefore we are seeking 2 nurses, so even though acute tasks transfer to the region, in the evening as a nurse you going forward will have good prerequisites to be integrated into and support the local home care groups and nursing homes, as well as the good citizen journeys in the home.

In the evening shift there are 3 nurses on duty together, covering the entire municipality and each taking the role of responsible nurse or base nurse.

The position of base nurses is what is available. In time, one can, upon request, be trained to become responsible nurse.

We have a good collaboration with TST (trans-sectoral mobile team) from Hillerød Hospital, for example with IV treatments or complex cases, where the hospital has the treatment responsibility. This collaboration is continually being expanded and as of 1/1 2027 will be the regional mobile emergency team we collaborate with.

We document in Nexus and use tablet and smartphone.

The 4 home care areas in the municipality (Nivå, Kokkedal, Fredensborg, Humlebæk) have implemented “self-managing teams”.

Typical tasks in the evening shift

• Visit and carry out planned nursing tasks.
• Palliative care pathways.
• Coordination and handling of suddenly arising tasks.
• Conduct assessment visits following referrals from external/internal partners as well as after discharge
• Administer IV treatment in collaboration with the region.
• Participate in triage meetings in the home care groups.
• Documentation.

We are looking for a new colleague who

• Enjoy providing nursing care to (typically) older citizens in the municipality.
• Is engaged, responsible, has a good sense of humor, flexible and proactive.
• Experience as a district nurse (home care nurse) and preferably from the secondary sector.
• Willing to provide nursing of the highest quality in collaboration with other professional groups.
• Takes an active part in developing the collaboration and professional sparring with the home care groups and nursing homes.
• Willing to take on the role of person in charge after training.
