About the job

Do you have high professional ambitions, and are you passionate about delivering high-quality nursing? Are you motivated to take clinical leadership and coordinate coherent care journeys for citizens in close collaboration with the home care service? Then perhaps you are the one we are looking for for our nursing group in Vallensbæk Municipality.

We are a small, independent nursing group with strong professional engagement and a shared desire to create the best possible pathways for the municipality's citizens. In Vallensbæk Municipality we work from an 'outward-in' perspective, where the citizen's wishes, needs and life situation are always the natural focal point of our efforts.

As a field-based nurse you will become a central professional resource in the organization. Based on the Elderly Act, you will be attached to a fixed home care team, where you have an important role in the nursing-coordinated care journeys in close dialogue with citizens, relatives and home care staff. The nursing group is self-referring, and you will therefore be responsible for assessing and initiating nursing interventions based on the Health Act, our service catalog and your own nursing competencies.

We are looking for a colleague who, together with us, wishes to strengthen learning, development and quality across the organization. You see the value in sharing your knowledge and contributing to both citizens and colleagues experiencing continuity, security and high professional quality. We work purposefully with early detection in close collaboration with the home care service and meet daily to ensure a common direction and coordination around the citizens.

What you get with us

You will become part of an organization where quality is a common ambition, and where cross-disciplinary collaboration is a natural part of everyday life. The nursing group consists of both younger and experienced nurses who handle a wide range of tasks including palliation, complex wound care and medication management.

We place a high priority on professional development. The group is led by a nursing professional leader, and weekly monocultural meetings are held with a focus on learning, sparring and quality development. Our nursing professional work is further supported by a quality consultant, documentation nurses and IT coordinators, who all have in-depth knowledge of practice and are closely involved in daily operations.

The implementation of the Elderly Act is well underway, and in the Center for Care and Welfare we are purposefully working to create coherent solutions for citizens. You will become an active part of the interdisciplinary collaboration with the home care service, prevention consultants, rehabilitation consultants, visitators, dementia coordinators and other partners. Together we ensure holistic care pathways with focus on quality, continuity and the citizen's needs. We all work from our common core task:

Through dignified relationships and high professional standards we, together with the citizen, create the possibility for the individual to live the best life every day.

We offer a thorough and structured onboarding process with individual introduction, so you get the best prerequisites to get off to a good start. In addition, you have great influence on planning your working hours through the wish plan in KMD Opus Vagtplan. Our MED committee actively promotes a good working environment, high well-being and a strong collegial community, because we believe that job satisfaction and professional quality go hand in hand.

We imagine that you

Are an engaged and reflective nurse with a focus on both dignified relationships and high professional quality. You are ambitious on behalf of the profession and are motivated to develop yourself, your colleagues and the organization around you.

You thrive in the role of clinical leader and naturally take responsibility for coordinating complex care journeys. You actively contribute to the nursing professional development, including guidance, sparring and teaching of colleagues in both nursing and home care. If you have experience with the improvement model, that is an advantage. Otherwise we would like you to be keen to get to know it.

You work systematically and on a professionally sound basis, gather relevant information and create the basis for good clinical decisions for the benefit of the citizen. At the same time you manage to keep an overview in acute and complex situations and have an eye for the details that make a crucial difference.

You place documentation high and see it as an important prerequisite for quality, coherence and continuity in the citizen pathways. In addition, you take active responsibility for a good learning environment and contribute positively to both the mono- and interdisciplinary collaboration.

Finally, you are responsible and a good colleague, contributing to a healthy working environment both professionally, physically and mentally.
