About the position

In Health Advisory we are looking for a nurse for our preventive initiatives for citizens aged 67 and above.

You will become part of a small team of three employees, which works purposefully to support older citizens' health, coping and participation in everyday life. The initiatives are based on the Elderly Act's framework for preventive efforts and mainly take place in the citizens' own homes.

You will play a central role in both implementing and further developing our preventive work.

Your tasks

You will among other things

• Conduct preventive home visits
• Work with screening and differentiated preventive interventions using Wipp 360
• Home visits when a spouse moves to a nursing home and upon concerns from relatives
• Conduct bereavement visits with citizens who have lost a spouse or cohabitant
• Support prevention of loneliness, falls and loss of functional ability
• Collaboration with associations and local communities
• Document, evaluate and develop interventions
• Engage in multidisciplinary collaboration with relevant actors in health and elder care

About Health Advisory

Health Advisory is part of the Health Department and consists of 18 health professionals with different disciplines, including nurses, physiotherapists, occupational therapists, clinical dietitian, PB in nutrition and health, and employees with an academic health background.

We work closely in multidisciplinary teams and emphasize high professionalism, knowledge sharing, flexibility and job satisfaction.

• Have experience in geriatrics, gerontology, psychiatry and/or preventive interventions

• Are proficient with IT and preferably have experience with CURA and Common Language III

Practical information

The position is 37 hours - other hours can be agreed. A satisfactory criminal record must be presented at the time of employment.

You will be employed as a self-directed employee under the applicable collective agreement, reporting to the head of Health Advisory, Christina Olofsson Sørensen.

The workplace is the Health Center in Odsherred Municipality, but a large part of the work takes place in citizens' own homes. Therefore a driver's license is required, and you must be able to provide your own car against travel reimbursement.
