About the position

In Health Advisory, we are looking for a nurse for our preventive measures for citizens aged 67 and older.

You will become part of a small team of three employees who work purposefully to support older citizens' health, coping and participation in everyday life. The initiatives are based on the Elderly Act's framework for preventive measures and primarily take place in the citizens' own homes.

You will play a central role in both the implementation and development of our preventive work.

Your tasks

You will among other things

• Carry out preventive home visits
• Work with screening and differentiated preventive measures using Wipp 360
• Home visits when a spouse moves to a care center and when there are concerns from relatives
• Conduct grief visits to citizens who have lost a spouse or partner
• Support prevention of loneliness, falls and loss of functional ability
• Collaborate with associations and local communities
• Document, evaluate and develop initiatives
• Participate in interdisciplinary collaboration with relevant actors in the health and elderly care sector

About Health Advisory

Health Advisory is part of the Health Department and consists of 18 health professionals with various disciplines, including nurses, physiotherapists, occupational therapists, clinical dietitian, PB in nutrition and health, as well as employees with an academic health background.

We work closely across professions and place emphasis on high professionalism, knowledge sharing, flexibility and job satisfaction.

Practical information

The position is 37 hours - a different number of hours can be agreed. Upon employment, a satisfactory criminal record check must be presented.

You will be employed as a self-managing employee according to the applicable collective agreement with reference to the head of Health Advisory, Christina Olofsson Sørensen.

The workplace is the Health Center in Odsherred Municipality, but a large part of the work takes place in citizens' own homes. Therefore a driving license is required, and you must be able to make your own car available with travel allowance.
