About the job
Are you passionate about public authority work and want to be part of an engaged and experienced team? Then you now have the chance to become our new colleague in the visitation team in Brøndby Municipality, as our colleague has pursued new challenges.
About us
Visitation consists of a total of 31 employees distributed across administrative staff, visitation, assistive devices, and caseworker teams as well as an assistive devices depot.
”We value professional quality in our case processing. Based on our professional knowledge we ensure timely help that supports the citizen's ability to live as independently as possible. Become part of a strong professional environment,” - Annette/visitator
The visitation team consists of 9 visitators who handle authority tasks within personal care, practical help, rehabilitation, care- and elder housing etc. In addition, we have 4 discharge coordinators who ensure good and coherent transitions from hospital to municipality. We process cases based on a holistic approach with basis in the municipality's quality standards, service descriptions and applicable law.
We are self-organizing and have great influence on planning our workday with the shared task solution. The visitator group is organized into smaller teams, reflecting the municipality's districts and areas of law within both the Elderly Act and the Social Services Act. This provides in-depth knowledge of the area’s home care groups, partners and citizens.
Interdisciplinary collaboration is a natural part of everyday life, and we participate weekly in interdisciplinary meetings together with home care, therapists, dementia coordinators and preventive staff to ensure cohesive pathways for citizens. We also hold regular dashboard meetings together with home care, where we monitor the development in care and welfare pathways and discuss citizens to ensure the right intervention, pathway type and a shared professional direction.
The vacant position is located in District East together with 2 experienced colleagues and you will be attached to a team responsible for citizens covered by the Elderly Act.
Vision
Our vision “Farther out – closer to” is at the core of our work. We believe that strong relationships and close collaboration with citizens, home care and other partners create the best everyday life for citizens.
Therefore we place high emphasis on communication and collaboration skills and actively work to build social capital across the organization.
Authority responsibility and role
We work based on the Elderly Act's intention of near visitation, self-determination and holistic pathways. It requires strong professional competencies, good collaboration skills and the ability to handle complex issues. At the same time, we are aware of our authority responsibility and the role as gatekeeper in relation to the municipality's initiatives.
“With us you get both influence and security. You will be heard and have the opportunity to influence future changes often with real co-determination. We stand together and help each other across, and you always have the management in your back. It creates a strong community and a secure everyday life, where we lift as a team..” – Dorte/visitator
