The 24-hour rehabilitation unit is urgently seeking an experienced social- and health care assistant for the new health- and care positions

At the health and care placements you will meet citizens who need more time, care, treatment and rehabilitation before they are ready to move on.

The citizens can have complex somatic and medical issues and several simultaneous diseases. Your clinical assessment, professionalism and ability to see the whole person will therefore be decisive.

You will have a daily life where you work independently and at the same time closely with other professional groups around the citizen.

The position is full-time, in rotating shifts and with start as soon as possible.

Your tasks

As a social- and health care assistant you will be a central part of the citizen's pathway. You will among other things:

• provide holistic care
• observe, assess, treat and follow up on the citizen's condition
• identify changes in the citizen's health and act on them
• collaborate with doctors, health professionals and relatives
• work closely with nurses, social- and health care assistants, physiotherapists and occupational therapists
• create coherence in the citizen's progression
• contribute to developing and implementing new work procedures.

As a result of the Health Reform, Varde Municipality's Day rehabilitation will transfer to the regions

1 January 2027 .

At Esbjerg and Grindsted Hospitals the new clinical department Citizen-Centered Health is under construction. The department will, among other things, accommodate health and care placements and aims to create a more close, cohesive and equitable health system.

You will be employed at Varde Municipality's Day Rehabilitation until 1 January 2027, after which your employment will transfer to the region.

Your starting point

Until 1 January 2027 you will be part of Varde Municipality's Day Rehabilitation , which currently has 20 rehabilitative placements.

Here we mainly receive citizens from the hospital for pathways that can last from one week to several months. The citizens have different somatic and medical issues and need rehabilitation to once again manage everyday life and achieve a meaningful daily life.
