About the job

We have a permanent position available in Rehabilitation and Welfare Technology.
Do you want to work rehabilitatively with a focus on the citizen's own goals while combining professionalism, welfare technology and close collaboration with home care? Then perhaps you are the one we are looking for in Rehabilitation and Welfare Technology at Nordfyns Municipality.
We work to enable citizens to live an independent, meaningful and dignified life for as long as possible in their own home – focusing on the citizen's own resources, active participation in daily life and involvement of civil society.
About the position
As a rehabilitation therapist in long-term pathways you will be part of a multidisciplinary team that works with:
- rehabilitation for citizens who receive long-term and lasting help
- competence development and professional sparring with the home care service
- implementation and use of welfare technology solutions, including DigiRehab
You will have your daily base in the home care group in Søndersø and participate in the close collaboration around the citizens, where coherence, continuity and a holistic view are crucial. In addition to daily collaboration, you will be part of the home care's various meeting forums, where you can contribute with your professional knowledge and knowledge about the citizens.
You will become part of a team of six occupational therapists and five digi-trainers.
As part of the implementation of holistic care, you will be part of a group with an important role in the operation. The group must continue to solve the current tasks and at the same time help develop work with near-visitation and follow-up closely to the operation.
Your core tasks
You work person-centered and rehabilitative and contribute to translating the municipality's quality goals and service goals into concrete practice. This means, among other things, that you:
- conduct goal conversations , where the citizen's perspective, wishes and resources are the focus
- make execution analyses of daily activities and plan activity-based rehabilitation interventions
- work according to the principle of the least intrusive help with a focus on self-reliance
- participate in near-visitation
- support that citizens can stay in their own homes for the longest possible time , among other things via welfare technology
- advise and spar with the home care service and contribute to competence development in rehabilitation
- function as a dignity ambassador as well as a facilitator at citizen conferences
- coordinate DigiRehab training courses
The interventions are planned based on the citizen's goals and may have restorative, compensatory or educational character, for example in programs focusing on energy management.
About you
We envision that you
- are educated as an occupational therapist
- have experience with or interest in person-centered rehabilitation
- can work systematically in the occupational therapy workflow
- enjoy working across other professional groups and taking the role of facilitator
- are curious, appreciative and good at seeing multiple perspectives
- can work independently and create structure in a varied daily life
- are proactive in your work
Knowledge of citizen conferences, the dignity journey team and welfare technology is an advantage – but not a requirement.
A driving license is required (if driving in your own car, compensation is according to SKAT's current rates).
We offer
- a meaningful job with broad professional breadth and influence
- thorough introduction to both the Adult Welfare area and the position
- opportunity to influence the ongoing development of the rehabilitation efforts
- close collaboration with engaged colleagues in the home care service
- therapeutic professional leadership and good monodisciplinary sparring opportunities
