About the job

We are seeking 2 occupational therapists for two completely new positions in holistic care

Would you like to be involved from the start – and have the opportunity to put your own mark on both the position, the tasks, and the occupational therapy approach in holistic care?

Then now is the time to read on.

Hedens Hjemmepleje is seeking 2 occupational therapists for two newly created positions in our holistic care.

The positions are new, and therefore you and your upcoming occupational therapy colleague will have a very special opportunity to help shape the function from the beginning. You will have the opportunity to influence tasks, workflows, forms of collaboration, and the way we integrate the occupational therapy and rehabilitative approach into daily home care.

We have the framework and the ambitions – but we want you to help create the content .

Who are Hedens Hjemmepleje?

Hedens Hjemmepleje is a choice-based provider in Herning Municipality, and we cover the entire municipality with personal care and practical help.

We started in November 2023 and today have about 90 employees.

Our core values are

Quality – Collaboration – Coherence

We have a strong focus on continuity for both our citizens and our staff. We believe that familiar faces, good relationships and close interdisciplinary collaboration help create quality and coherence in citizens’ trajectories.

With us, we prioritise visible and close leadership . There will always be a leader on duty and present, and we emphasise that issues are addressed as they arise – not necessarily waiting until the next day.

We help each other across functions and professional disciplines. No one is too big or too small to step in for another. Whether you are a field-based employee, therapist, coordinator or leader, we work together to keep everyday life running smoothly.

You will become part of a fixed team

As an occupational therapist you will be attached to either Team Land or Team By.

Team Land covers the northern, southern and parts of the west areas, while Team By covers the east and parts of the west.

In each team there will be approximately 12–18 field-based staff with whom you will have close collaboration.

You and the other occupational therapist will also have a close cross-team collaboration. You will mentor each other, help one another, and together contribute to developing the occupational therapy and rehabilitative efforts in Hedens Hjemmepleje.

In addition there will be professional meetings and opportunities for sparring with the municipality’s other therapists.

Your role in holistic care

As an occupational therapist you will be an important part of citizens’ overall care pathways.

You will not only work with the individual citizen but also help support our field-based staff in their daily work.

You will have a central role in relation to APV (work environment assessments), rehabilitation, assistive devices, functional capacity and follow-up on citizen pathways.

An important part of your role will be to ensure a rehabilitative approach to the citizen, to follow up on initiated measures, and to adjust or conclude pathways when the citizen’s needs change.

You will thus be both a professional sparring partner for the staff and an active part of the citizen’s pathway.

Your main tasks

Your tasks will include, among other things, to

· actively participate in multidisciplinary meetings, start-up meetings, citizen conferences, triage and other relevant coordinating forums

· contribute to planning and implementation of visited citizen pathways based on rehabilitation, the citizen’s own goals and a holistic approach

· evaluate and assess the citizen’s functional ability, activity execution, resources and rehabilitation potential, and initiate relevant occupational therapy interventions

· identify citizens with potential for rehabilitation pathways, including ABLE pathways, and carry these out in collaboration with Prevention and Contemporary Interventions

· assess the need for training aids, APV aids, welfare technology solutions and minor home adaptations, and contribute to coordination with the municipality’s authorities

· conduct home assessments in connection with hospital discharge or significant changes in the citizen’s functional capacity

· advise citizens, relatives and staff about assistive devices, activity-promoting methods, energy management, everyday mastery and appropriate workflows

· support a rehabilitative approach in holistic care and help ensure that the citizen maintains or develops the ability to manage as much as possible on their own

· ensure bridging to relevant training, activity and community offerings

· contribute to early detection of functional decline and initiation of relevant actions

· ensure timely, complete and professionally substantiated documentation

· contribute to staff development through teaching, professional sparring, guidance and on-the-job training

· participate in the development and implementation of rehabilitative methods, professional standards and workflows

· follow up on initiated actions and ensure citizen pathways are adjusted and concluded when goals are reached or needs change

· support field-based staff regarding APV and ensure good and safe workflows around the citizen

· participate in relevant professional networks, meetings and development activities.

Who are we looking for?

We are looking for you who are a trained occupational therapist and who want to use your professional skills closely with both citizens and staff.

It is a requirement that you have experience from home care and/or residential care, as the position requires a good understanding of daily operations, workflows and the complexity of care.

We also envision that you

· are passionate about rehabilitation and helping citizens achieve as much independence as possible

· thrive on working closely with other professional groups

· can translate your professional knowledge into concrete solutions in everyday life

· want to teach, guide and spar with staff

· can work independently while also seeing yourself as part of a shared team

· are flexible and can keep an overview in a day where plans sometimes change

· take responsibility and follow your tasks and citizen pathways through to the end

· want development and are willing to bring new ideas and proposals.

Two new positions – with room for your ideas

We have not created a finished box that you must fit into.

On the contrary.

It is the first time we hire occupational therapists directly into this function, and therefore we are looking for staff who want to help build the function together with us.

We know what we want to achieve: even stronger rehabilitation, better coherence in citizens’ pathways and closer professional support for our field-based staff.

How we best get there, we would like you to help shape.

Working hours and employment

The positions are primarily day shifts.

In addition, you will work every 6th weekend, where you will have a lead role to ensure an overview, coordination and coherence in weekend operations.

Start date: January 1, 2027.

Salary and terms of employment: Negotiated in connection with the interview and employment.

Application and interviews

Application deadline: November 6, 2026.

We expect to conduct first interviews on November 16 or 17, 2026.

There may potentially be a second interview on November 24, 2026.

If you are interested in one of the positions, please send your application to

[email protected]

If you have questions about the position or would like to hear more about the job and the possibilities of helping to shape the new function, you are very welcome to contact Andreas at tel. 41 34 99 48.

We look forward to hearing from you – and perhaps welcoming you as one of the first occupational therapists in Hedens Hjemmepleje.
