About the job
Introduction and framing
Would you like to help create coherent and meaningful rehabilitation pathways for citizens with chronic illness? Do you have a health professional background, and are you skilled at engaging people in their own health? Then this position may be for you.
In Halsnæs Municipality we are looking for a new colleague for our skilled team of health consultants, who are keen to hold clarifying conversations based on the individual citizen's life situation, coordinate interventions and provide instruction in our patient-centered prevention offers. We are looking for a colleague who is willing to actively contribute to the development of our patient-centered offers, and who focuses on creating greater health equity and promoting citizens' quality of life.
We are seeking a colleague who will join a small team of pathway coordinators, where each coordinator is responsible for interventions aimed at citizens with either diabetes, heart disease, COPD or cancer.
You will have a separate responsibility to develop and coordinate the municipality's initiatives within one or two diagnosis-specific areas, but you should also be willing to collaborate with your colleagues on the development of cross-cutting, generic offers and to hold clarifying conversations with citizens referred with an unspecified diagnosis.
You should be prepared to create solutions for the individual citizen that are based on the citizen's resources, wishes and life situation, and we expect you to be willing to involve relevant parties both inside and outside the municipality in the task resolution.
We expect you to see yourself in the following
- A mid-level health professional education such as nurse, physiotherapist or clinical dietitian
- Experience with development and operation of health-promoting initiatives – preferably with a focus on health equity and/or social vulnerability
- Knowledge of health-pedagogical tools and experience in applying them
- Good at communicating, building relationships and interacting with different citizen groups and actors
- Experience with motivational interviewing and experience in guiding and teaching citizens
- Experience with interprofessional work and preferably cross-sectoral collaboration
- Ability to work independently as well as participate in productive collaborations
If you also have experience working as a pathway coordinator, or facilitating group-based mastery programs, that is an extra plus.
About us
You will be employed in the Health Department, where we (with you) are 13 dedicated, competent and autonomous employees with varying professional backgrounds, working with a wide range of tasks within prevention, health promotion and rehabilitation. We are currently focused on implementing the Health Reform and clarifying our upcoming collaborations with the Region of East Denmark, with whom we have established a horizontal cooperation agreement concerning patient-centered prevention.
Additionally, we focus on developing health services targeted at children and youth, developing new mastery/management programs for, among others, citizens with chronic pain, and actively participating in a large partnership project with Steno Diabetes Center Copenhagen and DAB Lejerbo Frederiksværk, focusing on developing close health services for citizens with chronic illness.
In addition, we are responsible for the municipality's hygiene initiatives, brain injury coordination, dietitian functions, health preparedness, etc.
We offer you
- An exciting and varied job where you will have the opportunity to work with a wide range of stakeholders
- An ambitious workplace where there is room for humor, diversity and professional ambitions
- Skilled and engaged colleagues with broad expertise and experience in prevention and health promotion as well as a good knowledge of the health professional arenas in Halsnæs Municipality
- Opportunity for professional development
- Permanent office at the Town Hall in Frederiksværk with the possibility of ongoing remote work and influence over your own work scheduling
