About the job

Introduction and frame

Do you want to help create cohesive and meaningful rehabilitation pathways for citizens with chronic illness? Do you have a healthcare background and a flair for engaging people in their own health? Then this position might be something for you.

In Halsnæs Municipality we are looking for a new colleague to our skilled team of health consultants, who want to hold clarifying conversations based on the individual citizen's life situation, coordinate efforts and teach at our patient-oriented prevention offerings. We are seeking a colleague who is willing to actively contribute to the development of our patient-oriented offerings, and who focuses on creating more equality in health and promoting citizens' quality of life.

We are looking for a colleague who will join a small team of care coordinators, where each coordinator has responsibility for interventions targeted at citizens with either diabetes, heart disease, COPD or cancer.

You will have a separate responsibility to develop and coordinate the municipality's initiatives within one or two diagnosis-specific areas, but you should also be keen to collaborate with your colleagues on the development of cross-cutting, generic offerings and conduct clarifying conversations with citizens referred with an unspecified diagnosis.

You should be eager to create solutions for the individual citizen, based on the citizen's resources, wishes and life situation, and we expect you to be willing to involve relevant parties in and outside the municipality in the task solution.

We expect that you can see yourself in the following

  • Have a mid-length health-professional education such as nurse, physiotherapist or clinical dietitian
  • Have experience with development and operation of health-promoting interventions - preferably with a focus on equality in health and/or social vulnerability
  • Have knowledge of health pedagogical tools and experience in applying them
  • Are good at communicating, building relationships and interacting with different citizen groups and actors
  • Have experience with motivational interviewing and experience in guiding and teaching citizens
  • Have experience with cross-disciplinary work and also preferably cross-sector
  • Are able to work independently as well as participate in productive collaborations

If you also have experience with working as a care coordinator, or with facilitating group-based mastery programs, that is an extra plus.

About us

You will be employed in the Health Department, where we (with you) are 13 engaged, competent and independent employees with diverse professional backgrounds, who work with a wide range of tasks within prevention, health promotion and rehabilitation. Currently we are very focused on implementing the Health Reform and clarifying our upcoming collaboration relations with Region East Denmark, with whom we have entered into a horizontal cooperation agreement in relation to patient-centered prevention.

In addition, we also focus on developing health offerings aimed at children and young people, developing new mastery programs for e.g. citizens with chronic pain and actively participating in a large partnership project with e.g. Steno Diabetes Center Copenhagen and DAB Lejerbo Frederiksværk, where the focus is on developing close health offerings for citizens with chronic illness.

Additionally, we have responsibility for the municipality's hygiene interventions, brain injury coordination, dietitian professional functions, health preparedness, etc.

We offer you

  • An exciting and versatile job, where you have the opportunity to work with a wide range of actors
  • An ambitious workplace where there is room for both humor, diversity and professional ambitions
  • Skilled and engaged colleagues with broad professional expertise and experience in prevention and health promotion as well as good knowledge of the health professional arenas in Halsnæs Municipality
  • Opportunity for professional development
  • Permanent workplace at the Town Hall in Frederiksværk with the possibility of ongoing home working and influence on your own work planning.
