We did not find our new colleague at first, and therefore we try again! At Home Care and Nursing in Strøby we are looking for an engaged professional coordinator for the outreach area. The position is a key role, where you report directly to the manager and enter into a close, trusting partnership on the professional and resource planning.

The position is desired to be filled on 1 January 2027 (or as soon as possible).

Your main tasks and areas of responsibility

As a professional coordinator, your tasks range from strategic bridging to daily quality assurance:

• Coordination & discharges: You manage communication with hospitals, handle PFPs and coordinate discharges in close collaboration with the visitation team and the interdisciplinary back office
• Professional leadership & quality: You consult with your colleagues, support competency assessment of new employees, plan training and are responsible for the work with UTH
• Superuser in Cura: You share responsibility for documentation in Cura Care, including journal audits and on-the-job training of colleagues as well as teaching in our ambitious onboarding program
• Operations & planning: You ensure daily task planning in collaboration with the team’s nurses and step in as a substitute for the planners in home care when needed

Who are you?

We imagine you are a professionally grounded nurse with your heart in the right place and a eye for good structure.

• Experience: You are a licensed nurse with minimum 2 years of experience as a home care nurse in the primary sector
• Professional ballast: You have strong knowledge of the Social Services Act, the Elderly Act and the Health Act
• Strong communicator: You build bridges and communicate clearly and appreciatively with citizens, relatives and external partners
• Digitally oriented: You have a knack for IT solutions, and it is a big advantage if you already know Cura Care
• You have a driving license
