Nurse for a professionally exciting position in the District Psychiatric Centre Næstved

The position is an exciting nursing role where the focus is to offer patient care as long as possible in the local environment based on the F-ACT model.

Flexible outreach, proactive treatment in the local area (F-ACT) is a Dutch method for outpatient psychiatric treatment. The focus of the method is to be where the patient wishes to succeed. With F-ACT, there is a more multidisciplinary collaboration with the patient where the treatment intensity is flexibly adjusted to the patient’s needs.

The team’s target group is adults over 18 with conditions such as schizophrenia, psychoses, personality disorders, as well as adults with harmful use of substances and a concurrent mental disorder.

Through “defined pathways” that are agreed based on the patient’s wishes, hopes and expectations for the treatment and the possibilities of psychiatry, we seek to plan an individually designed and agreed course.

The District Psychiatric Service is part of the Psychiatry House - Integrated Psychiatry, which includes a high degree of collaboration with Næstved Municipality.

You will become part of a multidisciplinary team consisting of responsible and engaged staff with a high level of professional expertise.

In the District Psychiatric Service Næstved there are a total of 10 clinical contact persons (who are nurses, occupational therapists or social workers) who coordinate the treatment offers for patients in the target group.

In addition to the intensive F-ACT offering, the clinical contact person (among others) conducts group therapy and holds individually arranged conversations with patients and relatives.

The job involves a high degree of cross-sector collaboration, including participation in collaboration meetings as part of a targeted effort to create coherence in patient pathways.

Your task as a nurse in the F-ACT team is furthermore.

To be a clinical contact person for a rotating group of patients.

Home visits, outreach visits as well as virtual consultations

Sparring/ collaboration with your colleagues

Medication administration

Timely documentation in the journal.

Participate in quality and improvement work

Participate in teaching tasks related to patients, relatives and students.

We ensure you are prepared for the job through thorough introduction and for the first 12 months you will be assigned a mentor.

We have established monthly teaching and supervision.

Who are we?

F-ACT Team Næstved - Vordingborg provides outpatient treatment for referred citizens with long-term mental disorders as well as citizens with harmful use of substances and a concurrent mental disorder aged 18-75 within the target group.

Salary is set in accordance with the agreement between DSR and Region Zealand.

Starting salary for union employees

The region is obliged to inform you about the starting salary for the position. You will be informed of base salary, centrally fixed allowances and pension in the relevant collective agreement. In addition to the starting salary, the pay may include any negotiated allowances depending on the position, your experience and qualifications.
