Nurse responsible for general care

About the job

We are seeking a nurse for a home-visit team in Munkebo.

We are looking for an ambitious nurse who is either already or would like to develop into a truly competent generalist. You will be responsible for a wide range of nursing tasks. We constantly receive new types of nursing tasks from hospitals, and in home care the tasks are carried out under very diverse conditions in close interaction with the citizens and their relatives.

The team operates from Munkebo at Farimagsvej 52 and primarily visits citizens living in Næstved city centre or in Sct. Jørgens Park and surrounding areas.

The team is multidisciplinary and we work with the improvement model, hold daily triage meetings, where you as a nurse will also have a central role and are expected to apply your professional knowledge.

We work from the Elderly Act perspective with a focus on holistic care, staff continuity and the citizen's self-determination. Rehabilitation and prevention, early detection of dementia, acute nursing and palliation are our professional foundation. We work citizen-centered and are happy to go the extra mile to ensure the best possible effort for the citizens.

We offer citizens nursing care both in their own homes and in our nursing clinic in Munkebo.

To handle the increasing complexity and unpredictable daily life, we hold monthly citizen conferences and daily triage in the multidisciplinary mini-team.

We offer

A good working environment characterized by engagement and job satisfaction as well as professional discussions, peppered with pace and good humor.

We work in multidisciplinary teams, with mixed shifts, day and evening.

Opportunity to plan your own workday in close dialogue with your colleagues, and you will have great influence on how your duty roster will look; however you will be required to work roughly every 3rd weekend and a number of evening shifts.

-

You will have the opportunity to undertake a health-professional diploma education

You do not need to have experience from home care, as we provide a thorough introduction and you will be assigned to a mentor program.

Your tasks will include

  • Clinical assessment and follow-up of citizens
  • Acute tasks and palliative care
  • Wound care
  • Follow-up of medical treatment and development of nursing care plans
  • Multidisciplinary coordination and sparring – both internally and externally
  • That you think rehabilitative by involving the citizens in order to increase their motivations.

Your qualifications

  • Trained nurse.
  • Thrives with independent responsibility and a detail-oriented approach
  • Desire to share knowledge with the team and partners
  • Is solution-oriented and adaptable

It would be great if you have professional experience to draw on, but this is not a condition.

You must be able to support professional reflection, and at times step into a role regarding professional leadership.

If you want to contribute to the team’s professional development, that would also be a plus.
