Nurse with broad experience sought for the municipal palliative care team in Roskilde Municipality

The palliative team in Roskilde Municipality is seeking a nurse, 35-37 hours per week.

We work with complex nursing and therefore seek a nurse with a minimum of 5 years of total clinical experience and preferably experience in palliative care. The palliative team has been operating for a little over 5 years, and has embedded a common workflow that works in cooperation with authorities/ referrals and the other municipal services.

The palliative team consists of five nurses who handle the palliative care for citizens in their own homes in Roskilde Municipality. The team coordinates the overall effort between hospital/hospice, general practitioners, the regional palliative team and other health professionals, for the citizen in the last period of life. The palliative team functions as an professional advisor and sparring partner for the municipality's residential facilities, care centers, home care and rehabilitation places. The team participates in planning meetings with the discharge coordinator by agreement with the visitation.

The tasks in the team change quickly, and the nurses in the group are highly dependent on everyone being flexible and adaptable, and that people help each other to place and solve the tasks.

The palliative team works every 5th weekend and in day shifts.

The palliative team is organized under Home Care, District 2 in Roskilde Municipality, and is based at the Aster Center. There they share the premises with the rest of home care and nursing.

We are seeking a nurse who can

• Carry out health act interventions in accordance with existing guidelines and instructions for the palliative citizens in the terminal phase.
• Contribute to development efforts within palliative care
• Navigate a variety of situations and positions
• Be able to contain and manage the difficult crises for relatives when a loved one is at the end of life
• Be able to safeguard the citizen's wishes for the last period of life.
• Be able to communicate with citizens in crisis and have empathy for the situation.
• Dare to address the difficult questions that must be considered at the end of life.
• Have ongoing supervision
• Can coordinate pathways and collaborate with different units in the municipality
• Can involve relatives in the collaboration around the citizen.
• Have a driving license
