About the job

🏥 Nurse with the role of discharge coordinator

Orthopedic surgery department, ward 1, SUH Køge

Would you like to combine clinical nursing with coordination and take an important role in creating good and cohesive patient pathways?

We want to strengthen our discharges and therefore we are looking for a nurse who wants to work closely with patients – and at the same time be the one who creates an overview in the complex discharges.

With us you become part of the orthopedic daily life, while taking responsibility for coordinating patient pathways across hospital, municipality and the primary sector. You will be the one colleagues turn to when a pathway requires extra coordination, overview and planning.

🦴 Your tasks will include

  • Plan and manage complex discharges for patients needing expanded coordination, e.g. in particularly vulnerable or complex patients
  • Coordinate with municipalities, admissions/visitation, therapists and relatives in complex discharges
  • Create an overview of patient pathways and ensure progress in discharge plans, from admission to discharge
  • Collaborate with and educate colleagues about discharges and cross-sectoral pathways
  • Ensure that your colleagues have the knowledge and skills to handle less complex discharges
  • Participate in clinical nursing care and maintain close contact with patients

⭐ Why you should choose us

  • An exciting combination of nursing and coordination
  • Great influence on the patients' overall course
  • A strong community with high professionalism and good collaboration
  • Opportunity to develop both clinical and coordinating competencies and leadership
  • Opportunity to become a specialized orthopedic nurse within the specialties: wound care, ankle/foot and spine.

🩺 We envision that you

  • Have Danish authorization as a nurse – preferably with experience from the primary sector, but not a requirement
  • Thrive in the role of educator/role model
  • Thrive with responsibility, coordination and many collaboration interfaces
  • Can create structure and maintain overview in a busy daily life
  • Are motivated by both patient contact and interdisciplinary collaboration

🎓 Development and educational opportunities

We prioritise your professional development and provide concrete opportunities to build a specialized profile:

  • Thorough introduction and training in both orthopedic surgery and discharge coordination
  • Ongoing sparring and professional reflection in daily work
  • Participation in Region Zealand’s SKILLS program
  • Diploma education in orthopaedic surgery
  • Diploma module in clinical nursing care
  • Diploma education in wound nursing
  • Diploma module for clinical supervisor
  • Possibility of pursuing a professional master's degree (cand.cur.)

🕒 Working hours

  • Mainly day shifts with day/evening
  • Work corresponding to every 3rd weekend
