About the job

Ergotherapist or physiotherapist as prevention consultant for an exciting and versatile role

Would you like to contribute to strengthening the preventive effort for older citizens in Frederikssund Municipality? Are you a physiotherapist or occupational therapist and motivated by making a difference to citizens' ability to live an active, safe and independent life? Then we have an exciting opportunity for you.

Since our current prevention consultant will take on other tasks with us, we have had the opportunity to expand our therapy team, and we are therefore looking for an engaged occupational therapist or physiotherapist for a 37-hour position as prevention consultant in Prevention and Training as of 1 December or as soon as possible thereafter.

About the job

As a prevention consultant you will be an important part of our work to strengthen health, well-being and quality of life through the preventive effort for citizens aged 65 and older. You will play a central role in ensuring quality, continuity and early identification, so that more citizens can maintain their functional ability and live an independent life for as long as possible.

Your tasks will include among others to

  • Carry out preventive home visits to older citizens.
  • Advise and guide on health, well-being, activity, communities and support options.
  • Support citizens' own resources and ability to act with a view to preserving functional ability and independence.
  • Focus on preventing loneliness, illness and loss of function.
  • Facilitate contact to relevant offers, activities and communities in the local area.
  • Plan and conduct fall prevention courses.
  • Plan and hold joint events for new 75- and 80-year-old citizens in collaboration with activity centers and other partners.
  • Work with systematic identification and assessment of citizens, including the use of WIPP360.
  • Handle administrative tasks such as registration, documentation, sending out information and follow-up.

Prevention effort is part of the municipality's work under the Elderly Act and is based on an early, outreach and dialogue-based approach, where the goal is to meet the citizen where the citizen is.

You will be employed in Prevention and Training, where you will have therapy colleagues close by and good opportunities for professional sparring. At the same time you will have a high degree of independence and responsibility in daily work.

About you

We imagine that you are educated as an occupational therapist or physiotherapist and have an interest in prevention, health promotion and interdisciplinary collaboration around older citizens.

We emphasize that you

  • Have good communication skills and can build trusting relationships.
  • Thrive with independent work while valuing close teamwork.
  • Can work in a structured way and maintain overview in a varied daily life.
  • Have an interest in both citizen contact, teaching and administrative tasks.
  • Have experience with or an interest in preventive work among older citizens.
  • Are good at coordinating and collaborating with both internal and external actors.
  • Are familiar with IT and documentation or are eager to learn new systems.
  • Can see opportunities and resources in each citizen.
  • Have a driver's license and are willing to work across the municipality.

  • 1 development therapist

We have a strong professional community with a focus on high quality, development and collaboration. We work across two sites in Skibby and Slangerup, but solve tasks throughout Frederikssund Municipality.

You will be attached to both of our sites, but some tasks will take place with citizens or at different locations in the municipality. A car will be provided in connection with the job.

With us you will have the opportunity to become part of a committed team where we help each other, share knowledge and work together to create the best solutions for the municipality's citizens.
