Social- and Health Care Assistant - Home Care West - Herning Municipality

We are seeking a new colleague for a day shift position starting January 1, 2027 or by agreement.

With us you will become part of a close and strong multidisciplinary community, where your professional expertise helps contribute to holistic care, multidisciplinary collaboration and citizen involvement.

We work in fixed multidisciplinary teams, where continuity, presence and high quality in citizen care is at the center.

We are based at the Activity Center on Brorsonsvej and meet physically as well as via screen with nurses and therapists – making sparring and collaboration a natural part of our daily routine.

About the position

As a colleague in the team, you will help ensure continuity, presence and high quality in the citizen’s daily life.

The position is initially 37 hours per week, with work every 3rd weekend. We are flexible if you wish to have a lower number of hours.

Your tasks

• Ensure quality and continuity in citizens’ care and treatment
• Carry out tasks according to competence and delegation
• Coordinate and actively participate in our multidisciplinary meetings – you will be trained in CuraPlan
• Create an overview of citizens’ care pathways and contribute to planning of today’s tasks
• Engage in close collaboration with colleagues, nurses and therapists
• Contribute to professional sparring and development in everyday life.

Your profile

• You are trained as a Social and Health Care Assistant
• You are professionally well-grounded and can reflect on your own practice
• You are keen to take responsibility and work independently
• You have a good clinical eye and work from a holistic perspective
• You thrive in a coordinating role and in close collaboration with nursing
• You contribute to a good sense of well-being in the workplace.
