Social and Health Care Assistant for Home Care Skive/Højslev Night Shift

We are seeking a social- and health care assistant for the Home Care Skive/Højslev Night shift. Here you will be part of an independent and professionally strong night team, where good collegiality and the citizens' safety, dignity and self-determination are in the center.

With a basis in the Older Persons Act we work with holistic care, continuity and presence. We base our approach on what is important for each individual citizen, so the citizen can live a safe, dignified and as independent a life as possible in their own home.

About the night group

The night group handles planned and acute visits to home-dwelling citizens throughout Skive Municipality. We collaborate closely with the municipality's care centers and acute team and work purposefully with training and delegation, so the right competencies are brought into play for the right task. You will become part of an interdisciplinary team with high professionalism, a focus on development and a good working environment.

Your tasks

As a social- and health care assistant you play an important role in creating cohesive and safe pathways for citizens. Among other things you will perform personal care and assistance, health-care tasks, medication management, observation and documentation. You must be able to respond to changes in the citizen's condition, prioritize in acute situations and consult professionally with colleagues and partners. At the same time you support the citizen's own resources and involve the citizen's wishes in planning the help.

We are looking for someone who

is educated as a social- and health care assistant and thrives working independently at night. You are professionally curious, responsible and reliable, and you keep an overview when tasks change. You use your professional knowledge to assess, adapt and prioritize the help based on the citizen's current needs and see the value in close collaboration with colleagues, citizens, relatives and interdisciplinary partners.

You meet citizens with respect and understanding and actively contribute to a good working environment. With us we help each other, speak respectfully to one another and find solutions through dialogue and professional reflection.

The Older Persons Act in practice

We translate the intentions of the Older Persons Act into practice through holistic interventions and a rehabilitative approach. This means we support the citizen's self-determination and help the individual maintain or regain as much independence as possible. It requires professional reflection, good dialogue and courage to take responsibility close to the citizen's daily life.
