About the job

Sorø Municipality's nursing service is seeking a new nursing colleague.

  • Are you passionate about interdisciplinary collaboration and have a rehabilitative mindset?
  • Can you see yourself as a nurse in the citizen's own home with a focus on the whole citizen's everyday life/health issues?
  • Do you enjoy working in small teams and are a cheerful and positive team player with a good sense of humor?

Then this position is for you!

Special team is seeking a nurse in a permanent position 32 hours/week in varying shifts, day shifts (8-16), evening shifts (12-20) and 1 weekend shift every 6 weeks (time window 8-18). You can expect about 1 evening shift per week.

You must thrive in a daily routine where plans change quickly, depending on discharges and urgent tasks at hand. Therefore it is also important that you are broadly professionally grounded, as the tasks include parts of all specialties. You are, or will become, a specialist in being a generalist.

Special team’s primary target group is citizens who live in their own homes and receive home care and/or nursing services. We focus on reducing readmissions and creating security in discharge with preventive and coordinating efforts.

Special team also handles tasks in collaboration with the E-hospital, including IV medication.

The Special Team's tasks are

  • To provide holistic nursing with a high degree of interdisciplinary collaboration.
  • To conduct thorough nursing assessments.
  • To identify subacute issues and initiate preventive measures.
  • To assess potential for rehabilitative interventions and assess the citizen's functional ability in their own home.
  • Nursing tasks such as: medication dosing, medication administration, wound care, palliative interventions, measuring CRP, measuring vital signs (TOBS), INR testing, nutritional tube placements, fall assessments, safety programs, and acute tasks from general practitioners.
  • Documentation and interdisciplinary coordination constitute a substantial part of the work tasks.

About the Special Team

  • We are a team of 6 nurses and 1 physiotherapist.
  • We have close collaboration with outreach teams, the training unit, the home care service, the home care visiting team, the assistive devices team and general practitioners.
  • We stay up to date with the rest of the municipality's health services, as we must be able to advise and refer citizens to our colleagues' expertise when we hand the citizen over.
  • We are physically located in the Health Center and share premises with specialist nurses, nursing leadership, and nursing operations planners.

We offer

  • A joint introduction of 7 days and thorough training in the special team
  • A daily life characterized by many tasks around citizens, though there is room to develop skills and focus on well-being, as we place a high value on the working environment.
  • High degree of interdisciplinary collaboration. Selected visits are handled together with a therapist.
  • A good atmosphere with presence from management to staff and vice versa.
  • A workplace in a small municipality in central Zealand, where it is easy to reach by train, bus, and car.

About you

  • You are a trained nurse with at least 3 years of clinical experience
  • You are broadly professionally grounded and preferably with experience in geriatrics, cardiology, emergency department, abdominal/gastrointestinal surgery, or COPD
  • Experience with NEXUS is welcome
  • You are good at communicating orally and in writing with citizens and partners.
  • You place a high value on documentation to ensure continuity of care for the citizen in daily life.
  • You are a team player, responsible, and have an interest in your own and colleagues' joint professional development.
  • You have a driving license

Salary and terms of employment

  • The position is 32 hours per week
  • Day and evening shifts as well as weekend work
  • Commencement on December 1, 2026
  • Salary and employment terms are according to the applicable collective agreement and the principles of local wage formation.
  • Upon hire we will obtain a police certificate

Application deadline: October 23, 2026

Interviews will be held on October 28, 2026
