The new elder reform puts the citizen's wishes, resources and quality of life at the center. Therefore, we in Helhedsplejen Allingåbro work with coordinated citizen pathways, interdisciplinary collaboration and rehabilitative efforts, where the goal is to support citizens' ability to live an independent and meaningful life for as long as possible.

As a social and health care assistant, you will play a central role in this work. You will become an important professional close to the citizen and an active contributor in a strong interdisciplinary community.

Your tasks will include

• Holistic care based on the citizen's needs, goals and resources
• Execution of nursing tasks
• Medication management and follow-up on health-related interventions
• Key person for rehabilitative and preventive efforts in collaboration with the citizen
• Documentation and coordination in Nexus
• Close collaboration with nurses, therapists, home helpers and other partners
• Professional sparring and support for colleagues in daily work

We are looking for you who

• Are educated as a social and health care assistant
• Believe that the best care is created together with the citizen
• Have an eye for rehabilitation and the citizen's resources
• Are professionally engaged and curious about development
• Maybe have experience and interest in working with citizens with psychiatric diagnoses
• Can work independently and at the same time be a strong team player
• Can plan, prioritize and maintain overview in a varied daily life
• Document systematically and see the value of coherent citizen pathways
• Contribute positively to the work environment with responsibility, humor and helpfulness
