Svane Pleje Aabenraa seeks a social and health care assistant or social and health care helper for the evening shift to our outreach team

Svane Pleje is currently seeking a social- and health care assistant or a social- and health care helper with 28 hours per week in a 3-2 rotation and to work every second weekend in the evening shift for our department in Aabenraa.

We are looking for a colleague who can work independently and deliver the best care and support to our citizens. We offer a permanent position in our team of evening shifts with fixed shifts every other weekend.

As a social- and health care assistant or social- and health care helper in the evening shift you will provide care and practical assistance to our citizens in their own homes. At Svane Pleje we work rehabilitatively in close collaboration with citizens and relatives and do everything we can to create the best conditions for our citizens – with a particular focus on their life situation. We therefore have a strong focus on developing internal workflows and making time and space for collegial collaboration.

Participation in teams and planned time for sparring is therefore one of the everyday activities that we want everyone to take part in and be responsible for – and you will thereby be able to influence the team with your initiatives.

Primarily the tasks are

• Basic care and nursing tasks
• Practical help and personal care that supports the citizen in maintaining their dignity and creates joy of life
• Observation, testing and screening of citizens with a view to early detection and nursing
• Collaboration with citizens, colleagues and municipal partners
• Documentation in the care system KMD-Nexus
