Svane Pleje Horsens seeks a social- and health care assistant in day shift for our outreach team

Svane Pleje is currently seeking a daytime social- and health helper for our department in Horsens.

We are looking for a colleague who can work independently and provide the best care and support to our citizens. We offer a permanent 32-hour-per-week position in our day-shift team with fixed shifts every other weekend.

As a daytime social- and health helper you mainly perform tasks according to the Health Act and provide assistance to our citizens in their own homes. At Svane Pleje we work rehabilitatively in close cooperation with citizens and relatives and do everything to create the best framework for our citizens – with a particular focus on their life situation. We therefore place great emphasis on developing internal workflows and making time and space for collegial collaboration.

Participation in teams and planned time for sparring is therefore one of the daily activities that we want everyone to take part in and be responsible for – and through this you will be able to influence the team with your initiatives.

Job tasks are primarily

• Basic care and nursing tasks.
• Tasks under the Health Act.
• Practical help and personal care that support the citizen in maintaining their dignity and bring joy to life.
• Observation of citizens with a view to early detection of disease, with the aim of reporting it to our partners in the relevant municipality.
• Collaboration with citizens, colleagues and municipal partners.
• Coordination and planning functions.
• Documentation in the care system KMD-Nexus
