Nurse sought for Holistic Care, Vordingborg, East and West Team

Are you a nurse and would you like to help set the framework for Holistic Care in Vordingborg, team east and west, and at the same time be an active part of a strong multidisciplinary team, then you should read on.

We are currently undergoing a restructuring in Vordingborg Municipality and therefore need more professionally skilled nurses who want to help lift as a team. We are seeking professionally ambitious nurses who can deliver complex nursing services with high professional standards, who can take shared responsibility and contribute to a good working environment.

You will have influence and shared responsibility for your specific team. As a nurse in Holistic Care you will have close contact with all multidisciplinary collaborators. We work closely, among other things, with quality nurses to improve documentation, quality assurance and competence development.

We work with My Skills and My MedCard. We work very purposefully with Triaging in every shift change; we hold weekly wound triage together with SSA (social and health care assistants) and wound care nurses. Going forward, we will hold team meetings once a week to improve quality. We hold multidisciplinary collaboration meetings, where we focus, among other things, on the rehabilitative part of nursing. At these meetings, the assessment, assistive device consultants, physiotherapy, occupational therapy and clinical dietitian as well as our specialists in other specialties participate.

We are looking for you who are

• Registered nurse
• Possess extensive clinical knowledge and can act independently in complex situations
• Can communicate clearly, concisely and respectfully with both citizens/patients, relatives and colleagues
• Are curious, can share knowledge with other professional groups, and are development-oriented
• Are responsible and reliable.

Duties

You will facilitate triage, wound triage and team meetings. You will delegate tasks to our professionally skilled SSA and carry out complex citizen/patient pathways and ensure high-quality documentation. We document in the care system Nexus. You counsel and guide citizens/patients, relatives and other collaborators when delegating SUL services. You can work in a rehabilitative manner, where involvement and quality of life are at the forefront.

The Holistic Care in the Vordingborg area east and west will, in the long term, be consolidated so that we have the same intake point. At present, Team East is based at Rosenvang Nursing Home and Team West is based at Solvang Nursing Home.
