Nurse for Citizen-Centered Health, Acute Team in Skive – will you make a difference close to the citizen?

Citizen-Centered Health in Hospital Unit Midt is seeking nurses who want to be involved from the start in Citizen-Centered Health, where close-to-home health services should be shaped in practice. The department was established in connection with the implementation of the health reform to create more cohesive pathways, high professional quality and health closer to the citizens. Citizen-Centered Health will, from January 1, 2027, take on tasks that today are part of the municipal health offering.

Our core task is quite clear: we collaborate with citizens to strengthen health and promote the citizens' independent and meaningful lives. As a nurse with us you will have a central role in translating this core task into action – every day, in every pathway.

This is a permanent 37-hour-per-week position with employment from January 1, 2027 with mixed shifts, and we aim for you to work in two shift patterns – day/evening, day/night or evening/night with weekend shifts every 3 weeks.

As a nurse you will become a key person in creating progression in complex pathways, supporting the citizen's independence and strengthening health through professional quality and clear coordination. You will have a job where your nursing expertise is used broadly: in acute assessments, in rehabilitative work, in close dialogue with citizens and relatives, and in coordination with colleagues, other professional groups and partners.

The Acute Team Skive handles acute and special complex nursing tasks across the municipality and thus helps prevent inappropriate (re)admissions and ensures fast and qualified help after hospitalization.

As a nurse in the Acute Team you will have extensive contact with a wide range of partners, particularly municipal temporary placements, home care, municipal nursing, hospitals, on-call doctors and general practitioners.

The Acute Team is based in the Health House in Skive, where Skive Municipality and Hospital Unit Midt have a collaboration with a cross-sector clinical community between the municipality's nursing clinic and the hospital's acute clinic.

• are a licensed nurse and are motivated by citizen pathways where rehabilitation, acute nursing and complexity meet
• thrive in creating overview in complex and changing pathways
• can perform independent clinical assessments and prioritize in a busy daily life
• meet citizens and relatives with presence, respect and clear communication
• work independently but never alone, and have a willingness to change and a desire to help shape something new
• have experience with acute and complex pathways and can solve tasks at a high professional level

Salary and terms of employment

Salary and terms of employment are in accordance with the applicable collective agreement in the field.
