About the job
Are you an experienced social- and health care assistant who is passionate about care and can create an overview in everyday work?
Then we may have the job for you at Møllehuset.
You will become part of a close multidisciplinary collaboration with social- and health care assistants (SSA), practical nurses (SSH), social educators, nurses and our cross-disciplinary teams. With us, competence development and professional development are highly valued, and we hope you want to be an active part of our shared development.
A care coordinator with professional expertise close to the residents
As a care coordinator you will primarily have an active role in the care. You participate in the care on an equal footing with your colleagues, have your own care tasks and are close to the residents throughout the workday. At the same time, you have a special function in relation to creating overview and coordinating the day’s health-professional tasks in the team.
The care coordinator function is practice-oriented and an integral part of the daily care. You are present on the floor, where you both carry out care tasks, support your colleagues and contribute to ensuring that the day’s tasks are solved with high professional quality. This means you actively use your experience and professionalism in everyday life – both with the residents and in collaboration with your colleagues.
What does it mean to be a care coordinator?
You know the residents, their needs and daily life, and you use your professionalism to make the day’s tasks hang together.
You have, among other things, an eye for
- Which health-professional tasks need to be solved today?
- Who has the right competencies for the individual tasks?
- How do we get the SSA’s, SSH’s, nurses and students to work together as well as possible?
- Where is there a need for professional follow-up or action?
- Which situations require us to think preventively?
- How do we ensure quality, continuity and presence for our residents?
- How do we help each other to have a good and manageable workday?
You will become an important professional sparring partner for your colleagues and help ensure that the team’s total competencies are used in the best possible way for the benefit of the residents.
You lead the way – together with your colleagues
You succeed best in the role when you can see what is needed, can create overview, take responsibility and bring the right people around the task.
When the plan changes, you can stay calm, and at the same time have an eye for the most important: our residents and the quality they should experience in everyday life. As a professional role model, you dare to make decisions, ask questions and share your knowledge.
- We are looking for you who are educated as a social- and health care assistant
- have experience and professional depth – preferably from the elderly care sector
- can create overview without losing presence with the residents
- dare to take responsibility and make professional decisions
- communicate clearly, respectfully and appreciatively
- can see both the individual resident, the team and the whole
- work safely with documentation, reflection, follow-up and professional quality
- have a good mood and contribute to an open, honest and good working environment.
We offer
With us you get a good start with a thorough introduction, skilled colleagues and room to develop yourself. You get a varied daily life with influence on your work and the opportunity to put your mark on our shared everyday life.
We are a workplace in development with focus on professionalism, quality and well-being – and a management that listens and takes you seriously.
Welcome to Møllehuset
Møllehuset is part of De Gamles By and has 80 flats spread over four floors with 20 residents on each floor. After a longer period of extensive renovation, Møllehuset is now reopened, and almost all of our flats are already occupied. We are therefore in full swing to create a new everyday life and a strong community together with our residents and employees.
In connection with the reopening, Møllehuset has received 10 dedicated places for elderly deaf people, which enables us to develop a strong professional environment around this target group.
We have our own production kitchen and also run a bakery that supplies the five care homes in De Gamles By. We are a house in motion, where professionalism, development, community and decency matter.
Can you see yourself as a care coordinator with us?
If you are the social- and health care assistant who thinks: ”I want to be close to the residents and go into care together with my colleagues – and at the same time use my experience to create overview, coordinate and make the team’s professionalism work together,” we would very much like to hear from you.
