About the job

Are you the new team leader we are missing for Team Land?

Are you a trained nurse, professionally strong and good with people? Do you thrive with responsibility, an overview, and a daily life where no two days are the same?

Then it may be you we are looking for as our new team leader for Team Land at Hedens Home Care.

About Team Land

In Hedens Home Care, we are divided into Team City and Team Land, and we are now seeking a team leader who will take the daily responsibility for Team Land.

Team Land consists of about 12-16 fixed routes, covering the rural areas in Herning Municipality. On the routes, approximately 12-16 fixed employees work, as well as floaters who cover the employees' fixed days off.

You will become the team leader for a group of skilled and self-driven employees who know their fixed routes and have close and good relationships with the citizens.

Your most important task will be to be close to the employees and daily operations. You must listen to the challenges that arise, collaborate with the employees to find good solutions, and dare to lead from the front when the situation requires it.

A team leader who can both lead and participate in everyday life

We are looking for a team leader who is visible, accessible and present .

You must be able to create calm and an overview – even when everyday life changes quickly. You must be able to make decisions when necessary, but at the same time be humble, listening and open to the employees' experiences and input.

For us, good leadership is not just about sitting in the office. You must know your team, your routes and the citizens in your area. That gives you the best conditions to support the employees and make the right professional and operational decisions.

You will therefore also come out into care if needed. It is not a fixed part of your daily function, but can become relevant in cases such as acute illness, rapid growth or during holiday periods.

In other words, you should not be afraid to get involved when there is a need.

· are professionally strong and keep yourself updated within your field

· master written documentation in Cura

· preferably have knowledge of CuraPlan

· have good pedagogical and communication skills

· can support, guide and develop the employees professionally

· thrive with both administrative tasks and close contact with the employees

· work in a structured way and be able to keep an overview when there are many balls in the air

· communicate professionally and respectfully with citizens, relatives and partners

· can be clear and make decisions when the situation requires it

· while being listening, accommodating and good at collaborating.

Your areas of responsibility

As team leader you will have a central role in the daily operations and the professional leadership of Team Land.

Your tasks will include

· Planning and coordination of driving routes, where knowledge of CuraPlan and home care is an advantage.

· Having the daily overview of the citizens and the operations in your area.

· Ensuring continuity, the right competencies and necessary training on the routes.

· Following up on changes with the citizens and responding when their need for help changes.

· Handling dialogue and collaboration with the intake unit, cross-disciplinary partners, citizens and relatives.

· Being a visible and available sparring partner for the employees in daily life.

· Having responsibility for the professional leadership of the team.

· Handling the professional nursing responsibility in collaboration with our upcoming development and quality responsible nurse.

· Following up on citizen records and ensuring that the documentation is updated, relevant and functions as a useful working tool in daily life.

· Contributing to structure, overview and calm in daily operations.

· Assisting with phones and other tasks in the office.

· Collaborating closely with your team leader colleague and helping across when needed.

· Participating in care in cases of acute illness, growth, holiday periods or other situations where operations require it.

· Working every 6th weekend with a duty role.
