About the job

Are you an experienced nurse or social- and health assistant who builds trust in collaboration with relatives? And do you thrive in combining professional coordination with shift planning and close follow-up?

In Vordingborg Municipality we are seeking a team leader for a cross-disciplinary special team around a child with complex disabilities in their own home. The team consists of nurses, social- and health assistants and pedagogues.

About the position

You will become an important link between the family, the staff and the area manager. You must create coherence in the effort through clear communication, stable staffing and follow-up on agreements - with respect for the family's knowledge about the child and their daily life.

You will be based at the office in Langebæk and will not be part of the team's fixed care- and shift staffing. The position has no formal management responsibility. You will handle the daily coordination within the agreed framework and work closely with the area manager, who has personnel responsibility.

Your main tasks

As team leader you will among other things

  • be the family's professional contact person, align expectations and follow up on questions, agreements and changed needs
  • prepare rosters and coordinate holidays, shift swaps and absence with a view to continuity, working time agreements and the right competencies in the shifts
  • coordinate the cooperation between the family, the team and relevant professionals around the child
  • be a professional sparring partner, facilitate team meetings and support training and common working practices
  • follow up on professional quality including medication handling, documentation and task handovers.

The special team is your main task. After agreement with the area manager you also help with other shift planning and coordination tasks in Langebæk, including visiting and route planning.

Who are you?

We are looking for a licensed nurse or social- and health assistant with solid clinical experience from complex care and treatment processes as well as experience with shift planning. Experience with children and/or the disability field is an advantage.

You have experience with collaboration with relatives and can listen, build trust and maintain professional boundaries - also when there is concern or disagreement. You meet the family with respect for the fact that the help takes place in their home.

We also place emphasis on the fact that you

  • work in a structured way, follow agreements to the letter and keep overview when plans change
  • can bring together different professionalities and create follow-through without having personnel responsibility
  • have experience with professional sparring, documentation and quality work
  • have good digital competences and experience with shift planning and documentation systems. Familiarity with Nexus is an advantage.

We offer

You will have a specialized task where your contribution has great significance for the child, the family and the staff. You will become part of the collegial community in Langebæk and have a close partnership with the area manager.

Together with the team you will have the opportunity to develop the cooperation and the joint working practices around the child and bring both your nursing professionalism, your relational competences and your planning perspective into play.

Practical information

The position is 37 hours per week with base at the Langebæk office. Salary and terms of employment are in accordance with the applicable collective agreement.

Working hours placement and any on-call outside the planned hours are agreed separately.

Application deadline: 22 October at 12

Start date: 1 December 2026

If you have questions about the position, you are welcome to contact area manager Mette Suikkanen at phone 2169 9386 or area manager Sanne Toxværd contact at phone 2335 6224.
