About the job

Are you passionate about rehabilitation, complex nursing care and interdisciplinary collaboration – and would you like to help shape the future Health and Care Places? Then you may be the person we are looking for as a Team Nurse!

Exciting Career Opportunity: Become part of our Rehabilitation Team as a Team Nurse!

Are you passionate about rehabilitation, or do you already have experience in rehabilitative nursing? Are you enthusiastic about delivering complex nursing care in an interdisciplinary environment based on the citizen's resources and wishes? Then you might be the one we are seeking as Team Nurse in our interdisciplinary team at temporary daytime and nighttime rehabilitation placements – the future Health and Care Places.

An exciting future ahead

From January 1, 2027 we will become part of the Region's health structure, and that means a number of new and exciting opportunities for our team. We look forward to becoming part of the newly established Department for Citizen-Centered Health under Herlev Gentofte Hospital – a fantastic chance to be part of a journey where your role as a nurse becomes even more central.

Who are we?

Our Training and Rehabilitation Center is located in the heart of Lyngby in scenic surroundings and offers citizens temporary 24-hour rehabilitation placements, outpatient retraining, a mobile acute team, and the municipality's cross-disciplinary team of evening nurses. We have a strong interdisciplinary culture and good cooperation with both hospitals and the citizen's other networks. We are dedicated to helping citizens master their own lives, despite age and illness.

Your tasks as a Team Nurse

  • Together with your interdisciplinary team (SSA, occupational therapist, physiotherapist and assessor) coordinate goals, planning and evaluation of rehabilitation processes for each citizen.
  • Initiate, lead and perform treatment of multi-morbidity citizens based on your nursing assessment in consultation with the treating physician.
  • Lead daily board meetings and interdisciplinary conferences and ensure follow-up on agreements related to the individual rehabilitation courses.
  • Create a secure framework around the citizen and ensure close and constructive collaboration with both citizens and relatives.
  • Participate in the guidance of nursing students and SSA students.

We are looking for someone who

  • Broad clinical experience and preferably experience with rehabilitation and palliative nursing.
  • A systematic and evidence-based approach to nursing, based on health professional instructions.
  • Experience in coordinating and leading nursing care for multiple citizens, preferably as a coordinator of pathways, and coordinating with hospitals, general practitioners and home care.
  • Independence, overview and initiative in work, as well as a desire to work across disciplines.
  • Good collaboration skills and an appreciative approach.

We offer you

  • A thorough introduction with a fixed contact person, so you are quickly integrated into the team.
  • A strong mono- and interprofessional environment with good opportunities for sparring and professional development.
  • An engaged and responsible work community with a strong collegial spirit.
  • Focus on a healthy working environment that supports both your professional and personal well-being.
  • The chance to be part of an exciting journey with the transition to the new Department for Citizen-Centered Health and the development of our offerings in collaboration with the Region's health system.

Are you ready to take the next step in your career?

As a Team Nurse in the Training and Rehabilitation Center, you will have the opportunity to make a difference for the citizens of Lyngby-Taarbæk Municipality, while you develop professionally in an emerging area with plenty of opportunities.

Interested? We look forward to hearing from you!

Salary and employment conditions

The positions are initially 32-35 hours in daytime with weekend duty as responsible every 6th weekend. We are flexible and will try to accommodate your preferences.
