Fanø Municipality, Welfare, Health and Care, Nursing Team

Dear nurse – are you looking for new and exciting challenges?

We have a vacant position of 30-37 hours per week with work every 4th weekend. The hourly rate is agreed individually.

With us you start a work day with diverse professional challenges, close contact with citizens, and strong collaboration across Health and Care.

We create meaningful and coherent solutions for citizens based on Fanø Municipality's values: Responsibility, Respect and Meaning.

You will become part of a well-functioning nursing team with 12 professionally strong colleagues, who around the clock deliver municipal nursing care to citizens in their own homes, in care homes, at temporary placements and in the municipality's health clinic.

We are a dynamic team with humor, high professionalism and a strong commitment to both the citizens and to one another.

About the job

We work to create safe and coherent pathways for citizens with complex health and care needs. Based on the individual citizen's resources, wishes and life situation, we support health, coping, rehabilitation and quality of life.

The team handles complex nursing interventions, including acute pathways, prevention, early detection of changes in health status and basic palliative care.

As a nurse you will be responsible for contributing to continuity and quality in citizen pathways. In the role of contact nurse you will work with nursing assessments, planning, coordination and follow-up in close collaboration with citizens, relatives, general practitioners and other partners.

You will also play a central role in professional development through sparring, guidance and competence development of SOSU workers and other colleagues. Together we ensure high quality and an effort that is based on the citizen's needs, goals and wishes.

Basic palliative care is a natural part of our work. Therefore we expect you to contribute so that citizens with life-threatening illness and their relatives experience relief, security and dignity throughout the disease course.

You will also have the opportunity to bring your special skills and interests into play and help develop the future municipal nursing care in a multidisciplinary community with a focus on quality, learning and citizen value.

About the workplace

The Nursing Team is organizationally anchored in Health and Care and is based at Vangled 32 in Nordby. We currently share the address with Holistic Care. In the coming period, several of the municipality's departments will move to the address, including the Admissions Office as well as Social and Municipal Authority, which are expected to be in place within the next month.

We work in basic schedules with weekend duty every 4th weekend. The basic schedule also includes a few night shifts corresponding to three night shifts over four weeks.

We are seeking a nurse who

  • ideally has at least two years of experience as a nurse
  • is passionate about citizen-centered nursing and contributing to cohesive holistic care
  • can handle stable, complex and acute pathways
  • works independently and contributes constructively to both solo and interdisciplinary collaboration
  • communicates professionally and confidently with citizens, relatives and partners
  • can observe, assess and implement relevant nursing interventions
  • takes a holistic perspective on the citizen's situation and disease course
  • has a driving license

We offer

  • good opportunities for professional development and sparring
  • an individually tailored introduction and training program based on your experience and needs
  • engaged and skilled colleagues
  • an appreciative work community with proximity to both colleagues, management and partners
  • influence and the opportunity to shape the continued development of municipal nursing care in close collaboration with Holistic Care
  • a manageable organization where holistic view and short decision paths shape everyday life
