About the job
Dear nurse – are you seeking new and exciting challenges?
We have a vacant position of 30-37 hours per week with work every 4th weekend. The number of hours is agreed individually.
With us you start a workday with varied professional challenges, close contact with citizens and strong collaboration across Health and Care.
We create meaningful and coherent solutions for citizens based on Fanø Municipality's values: Responsibility, Respect and Meaning.
You will become part of a well-functioning nursing team with 12 professionally strong colleagues, who around the clock deliver municipal nursing to citizens in their own homes, in residential homes, in temporary placements and in the municipality's health clinic.
We are a dynamic team with humor, high professionalism and great commitment to both citizens and each other.
About the job
We work to create safe and coherent pathways for citizens with complex health and care needs. Based on the individual citizen's resources, wishes and life situation, we support health, coping, rehabilitation and quality of life.
The team carries out complex nursing interventions, including acute pathways, prevention, early detection of changes in health status and basic palliation.
As a nurse you will be responsible for contributing to continuity and quality in citizen pathways. In the role as the primary nurse, you will work with nursing assessments, planning, coordination and follow-up in close collaboration with citizens, relatives, general practitioners and other partners.
You will also play a central role in professional development through sparring, guidance and competence development of SOSU employees and other colleagues. Together we ensure high quality and an effort that takes the citizen's needs, goals and wishes into account.
Basic palliation is a natural part of our work. Therefore we expect you to contribute to ensuring that citizens with life-threatening illness and their relatives experience relief, security and dignity throughout the illness trajectory.
You will also have the opportunity to bring your particular competences and interests into play and help develop the future municipal nursing in a cross-disciplinary community with focus on quality, learning and citizen value.
About the workplace
The nursing team is organizationally anchored in Health and Care and is based at Vangled 32 in Nordby. Here we currently share the address with the Comprehensive Care. In the coming period several of the municipality's departments will move to the address, including Visitation and Social and Authority, which are expected to be in place within the next month.
We work on basic rosters with weekend shifts every 4th weekend. The basic roster also includes occasional night shifts amounting to three night shifts in four weeks.
We are looking for a nurse who
- would preferably have at least two years of experience as a nurse
- is passionate about citizen-centered nursing and contributing to coherent, holistic care
- can handle stable, complex and acute pathways
- works independently and contributes constructively to both mono- and interprofessional collaboration
- communicates professionally and securely with citizens, relatives and partners
- can observe, assess and initiate relevant nursing interventions
- takes a holistic perspective on the citizen's situation and disease course
- has a driving licence
We offer
- good opportunities for professional development and sparring
- an individually tailored introduction and onboarding process based on your experience and needs
- engaged and skilled colleagues
- an appreciative work community with closeness to colleagues, management and partners
- influence and the opportunity to shape the continued development of municipal nursing in close collaboration with Comprehensive Care
- an approachable organization where a holistic view and short decision-making paths characterize everyday life
