We are seeking dedicated and highly skilled nurses for new evening shift positions as visiting nurses in Sygepleje Vest with primary affiliation to the nursing homes

You will become a central part of a new arrangement for scheduling that will help shape and pave the way to ensure a coherent, flexible and professionally strong nursing coverage at the area’s nursing homes in the evening hours. You will be responsible for the area’s nursing homes, which are nursing homes in Stenstrup, Ollerup, Aldersro, Demensbyen, Tåsinge and Vindeby, where you contribute with nursing professional sparring, guidance and support to staff on shift. You will be part of the team of mobile nurses.

The positions have been created to foster a larger community and to strengthen the collaboration between nursing homes, home care and nursing, so that citizens experience high professional quality and coherence in their care pathways.

About the position

In the new evening shift position you will play a central role in the nursing professional effort at the nursing homes and in the collaboration with the mobile nurses. Your point of contact will be in the nursing department at Home Care West, where your nursing colleagues also report for work.

You will be on shift with two other colleagues who will travel out in Home Care West.

You work independently and continuously coordinate with staff and nursing colleagues and managers. At the nursing homes there are fixed daytime nurses available on weekdays, with whom you will also have a close collaboration.

You are responsible for, that

• you emphasize collaboration with citizens and relatives
• you are an important clinical support for staff and contribute to security, quality and development in the daily task execution
• you handle planned, acute and outreach tasks
• you perform nursing assessments and prioritizations
• you coordinate your own tasks in collaboration with the nursing homes
• you document performed interventions according to applicable guidelines
• you contribute to an interdisciplinary and cross-cutting collaboration between nursing homes and the mobile nurses
• you train and guide staff on the shift
• you relate to incoming care plan documents and discharge reports.

A strong cross-disciplinary collaboration

We want to strengthen the collaboration between nursing homes and the mobile evening shifts to ensure cohesive citizen pathways, high professional quality and a sensible use of nursing resources.

We have close interdisciplinary collaboration with relatives, pharmaconomists, dementia consultants, specialists, etc.

Nursing in Home Care West consists of 35 nurses, a good mix of age, experience and professional competencies. We are a well-functioning team with broad and high professional expertise, where collaboration, sparring and community are in the foreground.

We have weekly multidisciplinary meetings where we are a natural part of the overall assessment of citizens’ needs.

We have a professional and professional focus on the task solution for the citizens and the many partners around us. We work from a citizen-centric perspective and with a rehabilitative approach aimed at preventing hospital admissions.

We work with structured competence development, tailored to each employee’s wishes and professional interests. At the same time, it is a fixed part of the employment that you must complete a diploma module in rehabilitation and dementia.

In the nursing department there are various specialist functions including wound care, palliative care, Nexus, continence, clinical supervision and mentor for newly hired colleagues as well as various key roles.

Salary and employment

Salary and terms of employment according to the applicable collective agreement.

We expect our new colleague to start as soon as possible, or by 01.01.2027 at the latest. The position is in full evening shift with every second weekend worked.

The position is 37 hours, but we are flexible and will, as far as possible, accommodate your desired number of hours.

In Svendborg Municipality, the pay for this type of position typically ranges from DKK 37,534 to 54,773 per month for full-time employment. The pay includes pension, special holiday pay and fixed allowances. There may also be shift allowances and other special payments. It is important to note that your concrete salary is determined by your experience, seniority and competencies and negotiated with the relevant professional organization.

A private criminal record must be presented upon employment in the position.
