We are looking for

Nurse with the role of team coordinator in day shift.

Starting January 1, 2027

In connection with the implementation of the Health Reform, Svendebjerghave will be organizationally divided into a municipal and a regional department as of January 1, 2027.

Svendebjerghaves Municipal Temporary Overnight Beds will have room for 12 residents. The target group are citizens in need of a time-limited stay, where targeted care, rehabilitation, and/or respite care as well as support are provided with a view to creating the best possible conditions for the citizen's further progression.

We are looking for an engaged and professionally strong nurse with the role of team coordinator, who in close collaboration with the department head will help support the professional leadership and development of the new department.

The position is 37 hours per week in the day shift, Monday – Friday.

As Svendebjerghaves Municipal Temporary Overnight Beds become a satellite department to the care home Krogstenshave, you will have good opportunities for professional sparring with other team coordinators across the units. In addition, you will collaborate with the quality nurses at Krogstenshave.

Your primary tasks

As a team coordinator you will play a central role in coordinating patient pathways in collaboration with the other staff. You should also support professional quality and develop the staff's competencies.

Your tasks will include

• Manage planning of patient pathways to and from the time-limited overnight beds
• Receive the patients in the documentation system Cura and coordinate complex patient pathways together with the contact persons.
• Support the professional development of the staff through teaching, guidance and competence development.
• Train employees and approve relevant competence cards.
• Assume the role of student supervisor and support the training supervisors.
• Serve as internship supervisor for nursing students.
• Contribute to patient safety, including working with medication management and audits.
• Support the head of department in connection with ordering and purchasing nursing supplies.
• Assume the role as palliative care advisor and participate in the municipality's cross-cutting palliative care network.
• Actively contribute to quality development and implementation of new workflows.
