With us you meet citizens who are often clinically complex and not yet discharged. As a social- and health assistant you play an important role in creating stability, security and quality in citizen pathways, so rehabilitation succeeds.

You will be an important part of a multidisciplinary team, where you with your professional expertise observe, assess and respond to changes in citizens' condition and actively contribute to planning and follow-up on citizen pathways.

  • Nursing and health care tasks within the SSA's competence area
  • Medication management, documentation and wound care
  • Observation and follow-up on complex citizen pathways
  • Rehabilitative and holistic care focusing on the citizen's functional ability and goals
  • Coordination and collaboration around the citizen's overall rehabilitation efforts
  • Interdisciplinary and organizational collaboration with internal and external partners to ensure coherence and quality in citizen pathways
  • Professional sparring, knowledge sharing and development of the health care effort
  • To contribute to a safe and professionally strong environment where we help each other and share knowledge

We are looking for you who

  • Are educated social- and health assistant and passion to make a difference for citizens
  • Can handle complex health care tasks within your competence area
  • Are strong in clinical observations and early detection of deterioration
  • Work structured, independently and interdisciplinary
  • Have an eye for the connection between disease, treatment, care and function
  • Can take responsibility and act on relevant changes in the citizen's condition
  • Thrive in a changing everyday life and contribute positively to the team
  • See rehabilitation and holistic care as a natural part of your professional expertise
  • Are keen to help develop new working methods and create a strong professional culture

Whether you are experienced or newly graduated is less important. The important thing is your professional curiosity, your engagement and your courage to take responsibility.

We offer

  • A thorough introductory course
  • Nearby leadership and a strong professional environment with focus on sparring and knowledge sharing
  • Good opportunities for professional development and influence
  • Flexible shift planning with possibility for local agreements on working hours and shifts
  • A workplace in development with colleagues who help each other, so you never stand alone
  • A strong interdisciplinary collaboration with nurses, therapists and other professional groups
  • A unique opportunity to help shape the transition from municipal to regional operation and contribute to the establishment of the upcoming SOP places in Fredensborg Municipality
