About PLUS

PLUS stands for Pædagogisk Lokalt Understøttende Samarbejde and is a flexible §85-supported service under the Social Psychiatry in Fredensborg Municipality.

We work with citizens who have extensive psychological, social and cognitive challenges, and who require an intensive, flexible and individually tailored effort to thrive in their own housing.

Over the past two years, PLUS has developed into an offering with strong results. We experience that citizens can maintain their own housing, gain greater stability, increase their independence and avoid more intrusive interventions. Therefore PLUS is today an important part of the municipality's overall social-psychiatric effort.

We are looking for a colleague who

• Has a relevant social science education, e.g., pedagogue, social worker or social and health care assistant.

The everyday life in PLUS

No day is the same. One day you support a citizen in contact with psychiatry, the next you work on structure in the home, accompany to meetings or help create access to communities and meaningful activities.

You will become part of a team where we succeed together, help each other and share the responsibility for some of the municipality's most vulnerable citizens.

The practical framework

• We operate in a rolling schedule seven days a week between 08.00-21.00.
• We plan the visits together at the team meeting on Tuesdays based on the citizens' needs.
• The visits are scheduled flexibly within the 08.00-21.00 window to fit as well as possible the individual citizen.
• We drive out to the citizens, and the workday starts and ends essentially from home.
• We have a strong focus on documentation in our professional system Nexus
• We have a 13-hour shift that is rotated among employees.
• Working hours may vary, and you must be prepared for morning, evening and weekend work (every 6th weekend) year-round
• You must be able to use your own car in the job.

The position is 37 hours and to be filled as soon as possible or no later than 1 December 2026.
