When health professionalism meets humanity

We are looking for you who is a nurse or a social- and health care assistant, and who wants a daily routine with a calm presence at the forefront of your professional skills. We place the person behind the function high, and therefore you can be either a social- and health care assistant or a nurse, where we then adapt the framework, the responsibilities and the requirements according to your authorization type.

In other words, we are looking for you who can see the person behind the health care issues and at the same time have an eye for everything that must function around the citizen so that we succeed together in giving some of the most vulnerable citizens the best possible everyday life.

A daily life with both presence and professional responsibility

We believe that you should know the person you have the health care responsibility for. Therefore, you are every morning part of the care around the citizens, where you join the morning routines with your colleagues with both social and health care backgrounds. Here you meet the citizens and become part of the small routines, the personal encounters and the presence that gives you important knowledge about the individual citizen’s habits, needs and physical health condition.

It is also here that you get the opportunity to notice small changes, ask questions and react when something is not as it usually is. For us, the relational and the health care aspects are closely connected. They should create coherence in the health effort and ensure that we act on a professionally qualified basis.

The rest of the day you work exclusively as a coordinator and multidisciplinary health care professional regarding documentation, health care assessments, coordination, medication management as well as sparring and training of staff.

You are not alone

You become part of a multidisciplinary environment with close sparring from colleagues, management and the health care staff at the adjacent sheltered housing facility, Nøddevej 1. We expect independence and that you put your own mark on your everyday life so that it makes sense for the house, but not that you solve the task alone.

You also get an important collaboration interface to the associated protected activities and social interaction offering with an internal coordinator also attached. Here, health care coordination and knowledge sharing help create coherence in the citizens’ everyday lives so that we together support their health, well-being and quality of life and also can deliver health care initiatives while they are employed.

We are looking for you who work structurally, thrive in multidisciplinary collaboration and want to develop both your own and others’ professionalism.

Your concrete tasks include, among other things,

• perform health care tasks based on the citizens’ individual needs
• contribute to health care assessments, evaluations and action plans
• work with health promotion, prevention and follow-up on the citizens’ physical health
• support the quality of medication management and relevant health care workflows
• ensure relevant health care documentation and contribute to updating medication lists
• dispense medication
• follow up on unintended incidents in collaboration with management
• teach and train staff in health care tasks and medication management
• coordinate health care interventions and collaboration with doctors, hospitals and other external partners
• contribute to good handovers between sectors and perform coordinator roles for special tasks as well as admissions and discharges.

The position is, according to applicable law, posted as a full-time position of 37 hours per week. If you want fewer hours, this can be discussed. The hours are distributed for daytime shifts on weekdays.

Salary and employment are in accordance with the applicable collective agreement and local agreements, and we would like to point out that we will obtain a criminal record check before you are employed.

Salary range

• Social- and health care assistant 29,426 DKK - 34,717 DKK
• Nurse 32,416 - 39,424 DKK

Placement in the salary range depends on experience, competencies and relevant education.
