The Plejecenter Skovvangs somatic department is seeking a clinically strong nurse for day shifts.

Stillingen er på 32-37 timer ugentligt med opstart hurtigst muligt. Der er ingen weekendvagter og kun 2-3 helligdage årligt.

The position is 32-37 hours per week, with start as soon as possible. There are no weekend shifts and only 2-3 public holidays per year.

We are seeking a nurse with solid clinical experience who thrives with a high level of responsibility and can create structure, direction, and coherence in complex resident pathways. With us, you will be the lead-case nurse responsible for 20 residents and an important sparring partner for colleagues, doctors, and relatives.

Om stillingen

This is a position for you who want to work with complex nursing, coordination, and pathway responsibility. Your primary role is as the pathway-responsible nurse for 20 residents, where you ensure cohesive and coordinated pathways in close collaboration with colleagues, doctors, relatives, and other partners.

You will play a central role in the department's professional daily life, where you must be able to work independently, create structure, and keep a focus on quality in the residents' courses. You will become an important driving force in daily triage, departmental meetings, resident conferences, and other professional meetings.

You will be part of a nursing professional community with two nursing colleagues attached to Skovvangs' other departments, with whom you can continuously spar.

Your core tasks will be

• Forløbsansvar: Take on the overall nursing responsibility for 20 residents, including early detection of illness, prevention of unnecessary hospitalizations, and coordination of actions.
• Planlægning og opfølgning: Plan and coordinate treatment and care actions in collaboration with residents, relatives, general practitioners and hospitals, and ensure relevant follow-up.
• Faglig sparring og udvikling: Be a central sparring partner for social and health personnel, contribute to professional development, and support high quality in care.
• Oplæring og vejledning: Introduce, teach, and advise employees on nursing-related issues and guidelines.
• Tværfaglig koordinering: Actively participate in triage, resident conferences and interprofessional meetings, and ensure coherence in residents' pathways.
• Dokumentation: Ensure thorough and correct health-care documentation in CURA.

• Is a trained nurse and preferably has broad clinical experience.
• Has strong nursing skills and thrives with complex citizen pathways.
• Would preferably have experience with both somatic care and dementia or an eagerness to work across both areas.
• Thrives with independent responsibility and can create structure and coherence in complex resident pathways.
• Thrives in a guiding role and actively uses their professional expertise to support and develop colleagues' practice.
• Can maintain overview and act professionally in acute and unforeseen situations.
• Contributes actively to a positive work environment with openness and respect for each other's differences.
• Is eager to help develop nursing at Skovvang.

Salary and employment terms

Salary and employment are in accordance with the applicable collective agreement.
