Botilbud Elbæk in Hadsten is looking for a committed and professionally strong health professional coordinator who is passionate about working closely with both residents and staff.

The position is a combined role, where in the mornings you help support and accompany residents in the transition to day services for 10-15 hours per week, and the rest of the workday you handle the tasks as health professional coordinator. Working hours are weekdays 7-15, with possible meetings in the afternoons and evenings. About Botilbud Elbæk

Botilbud Elbæk is a dynamic and lively offering with room for 39 residents spread across 2 groups. The target group is adults with varying degrees of developmental disability who need support and help based on an individual pedagogical approach. Some residents have diagnoses in addition to developmental disability, such as autism, ADHD and anxiety.

Some of the residents are more or less self-sufficient when it comes to personal hygiene, undressing and dressing, and eating, while others need help with all practical and personal tasks. In this context transfers and personal care occur.

As a new employee at Botilbud Elbæk you will join a staff group consisting of experienced pedagogues, social and health care assistants, occupational therapists, social workers, psychomotor therapists, physiotherapists, care assistants, cleaning staff, group leaders and the department manager.

Our social-professional work is based on a rehabilitative approach, where we work from the residents’ individual action plans and sub-goals. Because of the residents’ functional impairments there is a sharp focus on health-related interventions, and in order to strengthen interdisciplinary collaboration, we want to hire a health professional coordinator. Your tasks

In the morning (10-15 hours per week)

• Support and motivate residents in connection with morning routines
• Carry out care tasks for residents who need them
• Help residents start well in day services
• Create structure, calm and security in the transition from home to day services
• Collaborate closely with the rest of the staff regarding residents’ needs

As health professional coordinator (22-27 hours per week)

• Coordinate and ensure quality in health-related interventions
• Support staff in health-related issues
• Ensure documentation and follow-up in relevant systems
• Contribute to the implementation of guidelines and procedures
• Collaborate with doctors, hospitals, relatives and other partners
• Participate in professional sparring and development of health-related practice
• Assist in teaching and guidance of staff at Botilbud and Day Services
• Prepare agendas for medication and staff meetings, have an overview and delegate tasks

We are looking for someone who

• Has a health-related education, e.g. nurse or social and health care assistant
• Has a strong health-professional profile
• Is structured and can maintain overview in a daily life with many collaboration interfaces
• Has experience with or a desire to teach colleagues
• Has good collaboration skills and a desire to contribute to shared development
• Has experience with documentation and preferably knowledge of relevant IT systems

Salary and terms of employment

Salary and terms of employment will be in accordance with the applicable collective agreement. Final salary appointment is determined after negotiation with the legally entitled trade union based on qualifications.
