About the job

Are you our new Home Adviser colleague?

Permanent position as a Home Adviser for the Neuro-pedagogical team

Are you the Home Adviser we are looking for. If you are passionate about working with citizens with acquired brain injury, we can offer a new colleague 37 hours per week, primarily daytime hours from Monday to Friday, in a well-functioning team. The position is 35-37 hours per week and is expected to be filled by January 1, 2027 or as soon as possible thereafter.

About us

The Neuro-pedagogical team is part of the Health, Care and Prevention Administration/SOFF in Odense Municipality under the program for Acquired Brain Injury. The staff is multidisciplinary and currently consists of pedagogues, occupational therapists, a social worker and a rehabilitation lead.

Our office is located at Kanslergade 8, and we are attached to the manager at Fællesboligerne Lille Glasvej, which is also for citizens with acquired brain injury. We are 9 employees with diverse professional backgrounds. We have assessed about 150 citizens aged between 18-75 across Odense Municipality.

Citizens are referred for temporary support, which may last for a shorter or longer period under § 85 and § 82B of the Social Services Act. The target group consists of people with acquired brain injury; some may also have psychiatric diagnoses and substance abuse issues.

The Home Adviser’s core task is to plan citizen pathways that support and develop the citizens' ability to master their daily lives according to their own wishes and dreams – despite their physical, cognitive and social impairments or challenges. This can be done in close collaboration with relevant partners in the municipality.

Our work with citizens is targeted toward making them as independent of us over time as possible, either with fewer visits or so that they can manage on their own.

The rehabilitative approach and the White Paper's definition of Rehabilitation form the basis for our work. We work in the documentation system Nexus with VUM. Where we base our starting point on the citizens' goals, sub-goals and actions.

About the position

You will become part of a team with broad experience in the neuro-pedagogical field and long experience with § 85 and § 82B support in their own home. The work is very independent and you are alone in direct contact with the citizen. As we take in more citizens with complex issues, we sometimes choose that there are 2 colleagues who collaborate around these citizens. You will be responsible for contact with 12-15 citizens.

Your work will consist of together with the citizens to plan rehabilitation programs that address the support needs for which the citizens have been referred. You will be supported in this by the team and the relational coordination we do together.

We support citizens based on individual assessments; whether we visit their homes, offer video calls, mobile calls, or they come to our office.

So that they become as self-reliant as possible.

The tasks can be of a very diverse nature and we must be able to support the citizen in understanding who to contact, e.g., a lawyer, a craftsman, various departments in the municipality, psychologist, companion

There may be practical tasks we support with at home such as dishwashing, tidying up, finances, conversations, etc

We often encounter tasks we have not faced before... and do not know the solution to.

Often there will be relatives at home who may be affected or in crisis due to the situation their loved one has found themselves in.

We expect that you have

Experience in interdisciplinary collaboration internally and externally.

Willingness to contribute to a good collaboration and work environment

Several years of experience and knowledge about the target group.

Must be able to use various digital platforms in IT, including PC, iPad, various mobile devices etc.:

The person we are looking for may be
