At the Center for Rehabilitation and Acute Care we are seeking as soon as possible a committed and professionally strong nurse and/or social and health care assistant who will contribute to developing and securing the health professional quality in the interdisciplinary collaboration. We work with the citizen at the center, and the rehabilitative effort is the pivot of our work.

The position is 28 hours per week in the evening shift.

Who are we At the Center for Rehabilitation and Acute Care we handle cases for citizens/patients with complex health care problems and needs for acute care, rehabilitation and temporary stays. This requires clinical nursing, coordination and an overview of health matters, professional assessment, systematic work, structure and documentation as well as the ability to act timely and safely.

We are a team consisting of nurses, social- and health care assistants, social- and health aides, therapists, house assistants, social workers, students, and substitutes, who work closely with and for the individual citizen.

About the job In the position you will have a broad portfolio of tasks ranging from acute to planned nursing interventions. You will be responsible for observation, assessment and follow-up in citizen pathways and actively contribute to daily interdisciplinary coordination.

Your primary tasks will include, for example

• Assume and coordinate nursing care for citizens/patients with complex and changing health needs
• Carry out systematic clinical observations, professional assessments and relevant actions when the citizen's condition changes
• Ensure accurate and comprehensive health documentation, including follow-up on the 12 nursing problem areas
• Work systematically with medication management, patient safety, risk assessments and prevention of adverse events
• Contribute to ensuring that applicable instructions, workflows and professional standards are followed in practice
• Support colleagues through professional sparring, guidance and sharing of knowledge
• Actively participate in the interdisciplinary collaboration with social- and health care assistants, therapists, doctors, hospitals and other partners
• Contribute to rehabilitative pathways where the citizen's/patient's plan, resources, goals and potential for maximum independence are central

The workday is varied, and you will meet both citizens with acute needs and citizens in longer rehabilitation trajectories.

We operate with a flexible shift plan, which means you have influence over the placement of your shifts.

• Are trained as a nurse or social- and health care assistant and have a solid clinical foundation or ambitions to develop professionally toward more competencies
• Have strong clinical competencies and can identify, prioritize and respond to changes in citizens' health status
• Work systematically with documentation, medication handling and patient safety
• Can translate professional instructions and guidelines into safe practice
• Have good collaboration skills and can communicate your professional knowledge clearly and respectfully
• Take responsibility, maintain an overview and contribute constructively to professional development
• Wish to support colleagues in maintaining and developing high health-professional quality
• Have strong professional motivation and want to actively contribute to the development of practice, quality and patient safety
