About the job
The primary purpose of the forthcoming health reform is to strengthen the primary sector, and more health tasks will in the future be carried out closer to the citizens' own homes. A new quality standard for municipal nursing care and basic palliative care will be implemented, which places increased requirements on municipal nurses' clinical decision-making competence, clinical leadership, and ability to create coherent care pathways across sectors.
Therefore, the Nursing Unit in Esbjerg Municipality is creating new positions as clinical competency nurses, who will help lift complex nursing tasks and support upskilling in the rest of the municipal nursing care. This is a developing role where you will be able to act autonomously in the specific citizen contact and contribute with professional sparring, knowledge sharing, and training in the rest of the municipal nursing care.
We are seeking nurses with a solid clinical foundation who can handle complex and unpredictable citizen pathways, identify early signs of deterioration, and translate clinical observations into relevant actions. You must be able to navigate safely in transitions between hospital, general practice, and municipal nursing care, where timely follow-up, coordination, and clear communication are crucial.
The positions are generally 37 hours per week with day and evening shifts and occasional weekend shifts.
Start date 1 January 2027.
To ensure a uniform development of the positions, during the start-up period you will report directly to the Head of Nursing and have a joint onboarding with the other clinical competency nurses. You will also become an integrated part of the rest of the municipal nursing care, where you in your daily work will be permanently attached to and have your daily work in one of the nursing unit's five mobile teams.
Your tasks will include, among others
- Independently manage and support complex citizen pathways, including citizens with multimorbidity, unstable health conditions, or the need for special attention after discharge.
- Conduct systematic clinical observations and assessments with a view to early detection of illness and deterioration.
- Respond to inquiries from general practitioners, other municipal nurses, and social and health assistants if there are concerns about a citizen's health condition.
- Initiate relevant nursing actions and follow up on their effects with a focus on early prevention of unnecessary hospital admissions and readmissions.
- Identify nursing-related problems and points of attention during sector transitions, including the need for observation, treatment, medication follow-up, and clear allocation of responsibilities.
- In close collaboration with the rest of the municipal nursing care, coordinate and follow up on quality challenges that affect citizen safety in cross-sectoral pathways.
- Be a professional sparring partner for other municipal nurses and contribute to training and competence development in concrete nursing tasks.
- Perform relevant clinical procedures, including establishing IV access, and support the professional development of other municipal nurses in the area.
- Contribute to ensuring that new knowledge, changed treatment needs, and new tasks can be handled safely in the municipal nursing care.
Tasks will be developed in step with the implementation of the health reform and the needs that arise in collaboration between municipal nursing care, general practice, and the region.
Expectations of a future colleague
We envision that you have a solid clinical foundation and experience in handling complex nursing-related issues in multimorbid patients.
We place particular emphasis on the fact that you
- Have experience from hospital, municipal nursing care, or another relevant clinical role where you have worked with complex and unpredictable patient and citizen pathways.
- Work systematically and structurally with clinical observations, e.g., TOKS, and have experience with relevant clinical procedures.
- Have strong clinical assessment skills and can observe, analyze, and respond to changes in the citizen's condition.
- Can maintain an overview and prioritize relevant actions, even when the situation is complex and there is no single clear solution.
- Have an understanding of the clinical and organizational challenges that can arise at sector transitions, and can identify the need for follow-up and coordination.
- Communicate clearly and collaborate constructively with different professional groups internally and across sectors.
- See the citizen's overall situation and be able to connect somatic issues with other factors that are important for a safe and coherent citizen pathway.
It is a condition that you have a driver's license.
We offer you
- Employment in a trust-based organization with professional leadership.
- To become part of an organization in development, where your clinical assessments, your professional judgement, and your ability to create coherence can have a significant impact on citizens' pathways.
- A role with the opportunity to apply your strong clinical competences in close collaboration with colleagues in the rest of the municipal nursing care.
- Opportunity to influence the development of a new structure and new working procedures in connection with the transition to a strengthened primary sector.
- Close collaboration with municipal colleagues, general practice, hospitals, and regional partners.
- Opportunity for relevant further education in the clinical field as well as specialized training in citizen-centered nursing.
