About the job

Do you want to help bring the citizen's needs, employees' competencies and our resources to fit together in everyday life?

We are at the heart of one of the largest and most exciting changes in the health and elderly care sector. With a basis in the Elderly Care Act, we organize ourselves into fixed cross-disciplinary teams, where the citizen's wishes, resources and life situation are the starting point for the support and help we provide.

At the same time we are implementing our new governing logic, the Borgerrejsen (Citizen's Journey). The ambition is clear: Citizens should experience coherence, influence, and support to live an independent and meaningful life for as long as possible.

Across Health and Elderly Care we are also working with a cultural movement where we move from responsibility to shared accountability. We must not only succeed with each of our parts of the task. We must succeed together for the citizen's entire journey.

Here you, as a resource coordinator, will have an important role. You will become a central driver in daily coordination and planning and must help create coherence between the citizen's needs, the team's competencies and the resources we have at our disposal.

Therefore we are now seeking resource coordinators for the Borgernær Indsats. Your role as resource coordinator

As a resource coordinator you will have a central role close to the citizen-facing team. You will help create an overview and coherence in everyday life and support that citizen pathways are translated into concrete, coordinated and professionally sound actions.

You will work closely with the team, the department head and the district manager and will also be an important link to, among others, the intake/assessment and other partners around the citizen's journey.

A significant part of your task will be to align the citizen's needs, the employees' competencies and the available resources. You must be able to see both the day's needs and the longer-term perspective and help create a plan that supports continuity and quality for the citizen. Your main tasks

As a resource coordinator you will among other things

  • coordinate and support citizen pathways across home care, nursing, and occupational therapy based on the citizen's goals and self-determination
  • support daily planning and allocation in CuraPlan and continuously adapt the planning when needs and operations change
  • work systematically with master plans and day-to-day planning with a focus on continuity for citizens
  • support that tasks are distributed according to the right competencies, and that delegation is done correctly
  • contribute to interdisciplinary coordination including triage, citizen conferences and ongoing dialogue with the team
  • support collaboration with the intake/assessment and other partners around the Borgerrejsen
  • contribute to coordinating competence development and knowledge sharing within the team
  • follow up on quality, planning and documentation
  • support empowerment and shared accountability in the team
  • contribute to stable operations with a focus on quality, well-being and the use of our combined resources
  • opportunity to take on shift planning in Optima in close collaboration with management
  • opportunity to take on the role of meeting facilitator in different meeting formats

You will also maintain a close connection to practice and will participate in citizen-facing operations to some extent. It is important to us that you know the reality you are helping to coordinate. Your participation in operations must be balanced so that you can continue to perform your core task of coordination, planning and overview. Who are you?

We are seeking someone with a health professional background who thrives in a daily life where planning, coordination and interdisciplinary collaboration go hand in hand.

We value that you

  • have an overview and work in a structured way, even when daily life changes
  • communicate clearly and collaborate well with different professional groups
  • can prioritise and translate needs into action
  • have an eye for the connection between tasks, competencies and resources
  • have insight into delegation and interdisciplinary collaboration
  • have knowledge of Cura and CuraPlan
  • are curious about new ways of working and organizing ourselves
  • are eager to take an active part in implementing Borgerrejsen and developing the fixed interdisciplinary teams
  • can navigate changes while contributing to calm, direction and structure in daily life
