In Hørning Hjemmeteam Team 2 we are looking for a skilled and engaged social- and health assistant who wants to be part of a community where people meet people, and where citizens' wishes, hopes and dreams are the starting point for our work.

With us you become part of Team 2, which, together with Team 1, covers Hørning town. We are a smaller interdisciplinary team with a strong sense of camaraderie and a culture shaped by trust, professionalism and responsibility. At the same time, we are part of a larger house with three interdisciplinary teams, where collaboration and knowledge sharing across teams is a natural part of everyday life.

We believe the best solutions are created close to the citizens and by the employees who know them best. Therefore we have co-ownership of planning our everyday life and are involved in planning the routes based on the citizens' wishes and needs as well as our professional assessments.

We work rehabilitatively with a focus on the core task: To support citizens in living an independent and meaningful life for as long as possible.

Your tasks will include

• To perform complex health-related tasks for citizens
• To contribute to rehabilitative citizen pathways with a focus on resources rather than limitations
• To ensure coherence and quality in citizen pathways
• To collaborate closely with citizens, relatives and colleagues across professional groups
• To actively contribute to the development of the team's professionalism and workflows
• To support students, new colleagues and professional sparring in daily life
• To take co-responsibility for planning and coordination in the team

• Are educated as a social- and health assistant
• Have knowledge of home care or a desire to become part of it
• Have an interdisciplinary and rehabilitative mindset
• Thrive with responsibility and want to contribute to the development of the operation with the citizen at the center
• Are curious, solution-oriented and can work independently
• See the value in co-ownership and the shared responsibility for the team's tasks
• Understand that the most important task is to know the citizens and create relationships that make a difference

You should expect to work every 3rd weekend and have about 6 evening shifts in 12 weeks.

The position is initially 37 hours per week and start by agreement.
