Nurse with flair for coordinating citizen journeys sought for Glostrup Municipality's new respite care beds

Do you have a natural talent for creating cohesive citizen journeys, coordinating complex efforts and collaborating with many different actors? Do you want to help shape a new function with great professional influence? Then this may be the position for you.

An exciting new assignment

As of January 1, 2027, the municipalities' health and care facilities will transfer to the region. In Glostrup Municipality the respite beds will be continued, where citizens can be admitted for a shorter stay under the Elderly Act §16 and 17 and the Social Services Act §84.

We are establishing 6 respite beds for citizens who need a temporary stay due to special care needs or to relieve relatives.

We are seeking a nurse who can be the ongoing coordinator and ensure high professional quality in the citizens' journeys.

Your tasks

You will play a central role in the setup and operation of the respite beds. Your primary tasks will be to:

• Serve as the coordinator of citizen journeys from referral to discharge.
• Ensure coordination and collaboration between relevant partners.
• Involve and work closely with the citizens' relatives, who play a central role in the process.
• Collaborate with the GP, hospital departments, admission, home care, home nursing and center nurses.
• Ensure coordination of the beds.
• Ensure record-keeping, documentation and collection of relevant information.
• Plan the citizen's further pathway after the stay in close cooperation with admission and relatives.
• Ensure and support the staff's professional development.
• Contribute to the development of a department with high professionalism, respectful communication and a good team environment.

You will also become part of the center nurse network, which consists of the nine center nurses from Glostrup Municipality's three elderly centers. We have a development nurse attached to the elderly centers who provides professional sparring and supports development initiatives.

Workplace

In the startup phase, the respite beds will be located at Digevang, where the municipality's health and care beds and respite beds have previously been gathered.

It has been decided that the respite beds will move to newly renovated premises at Elder Center Dalvangen late in 2027. Dalvangen is directly connected to Digevang and connected via a glass corridor.

• Has experience coordinating complex citizen journeys or solid knowledge of the primary sector
• Works in a structured way and has a strong overview

• Wants to contribute to professional development and training of colleagues
