Nurse for Citizen-Centered Health, Acute Team in Skive – do you want to make a difference close to the citizen?

Citizen-Centered Health in Hospital Unit Midt is seeking nurses who want to be involved from the start in Citizen-Centered Health, where close-to-home health care should be shaped in practice. The department has been established in connection with the implementation of the health reform to create more cohesive care pathways, high professional quality, and health care closer to the citizens. Citizen-Centered Health will, from January 1, 2027, take on tasks that today are part of the municipal health offering.

Our core mission is quite clear: we collaborate with citizens to strengthen health and promote their independence and meaningful lives. As a nurse with us you will play a central role in translating this core task into action – every day, in every care pathway.

This is a permanent position of 37 hours per week with employment from December 1, 2026, with mixed shifts, and we aim for people to work in two shift blocks – day/evening, day/night, or evening/night with weekend shifts every 3 weeks.

The Acute Team Skive handles acute and specially complex nursing tasks across the municipality and thus helps prevent undesired (re)admissions and ensures quick and qualified help after admission.

As a nurse in the Acute Team you will have extensive contact with a wide range of partners, in particular municipal temporary placements, home care, municipal nursing, hospitals, on-call doctors, and general practitioners.

The Acute Team is based in the Health House in Skive, where Skive Municipality and Hospital Unit Midt have a collaboration with a cross-sector clinical community between the municipal nursing clinic and the hospital’s acute clinic.

We are looking for someone who

• is a licensed nurse and is motivated by citizen pathways where rehabilitation, acute nursing and complexity meet
• thrives on creating an overview in complex and dynamic pathways
• can perform independent clinical assessments and prioritize in a busy daily life
• meets citizens and relatives with presence, respect and clear communication
• works independently but never alone, and is willing to change and eager to help shape something new
• has experience with acute and complex pathways and can solve tasks at a high professional level

Salary and terms of employment

Salary and terms of employment are in accordance with the applicable collective agreement in the field.

With reference to the Act on Child Certification in connection with the employment of staff, a child certificate will be obtained in connection with any possible employment.
