About the job
Are you a physiotherapist or an occupational therapist, and are you passionate about strengthening citizens' ability to live an independent life for as long as possible in their own home?
In Døgnplejen Frederikssund we work every day to create cohesive pathways based on citizens' own goals, resources and wishes. With the new Elderly Act, rehabilitation, training and holistic care will take an even more central role, and therefore we are looking for a therapist who will help develop and strengthen the rehabilitative efforts in our multidisciplinary teams.
With us you become an integral part of daily life in Døgnplejen, where therapists, nurses, social and health care assistants, social workers, planners and other professional groups work closely together on shared citizens and shared goals.
Your duties
As a therapist in Døgnplejen you work closely with the citizens and with your colleagues.
You plan and implement rehabilitative processes in the citizens' homes, but an important part of your work is also to support the rehabilitative mindset in daily practice. You contribute with professional sparring, guidance and competence development so that rehabilitation becomes a natural part of holistic care.
You will, among other things
- conduct rehabilitation and training programs in the citizens' own homes
- identify citizens with potential for increased self-reliance
- be a professional sparring partner for colleagues in the multidisciplinary teams
- provide guidance on ADL, aids, transfers and functional capacity
- support work with citizens' goals, motivation and mastery of everyday life
- contribute to cohesive and coordinated citizen pathways
- help translate the Elderly Act's intentions of holistic care, rehabilitation and citizen involvement into practice.
We are looking for someone who
- is trained as a physiotherapist or occupational therapist
- has an interest in or experience with rehabilitation in the elderly field
- thrives in close multidisciplinary collaboration
- is curious, proactive and visible in everyday life
- can translate your professional knowledge into practice-oriented sparring and guidance
- is motivated to create development for both citizens and colleagues.
Why you should choose us
In Døgnplejen Frederikssund you will have the opportunity to influence the development of future elderly care. You will become part of an organization where multidisciplinary collaboration, professional development and citizen involvement are fundamental values, and where we believe that the best solutions are created close to the citizens and in strong professional communities.
We work to enable more citizens to live the life they want as long as possible in their own home. Do you want to be part of that?
Your area of work
You will be employed with attachment to Døgnplejen Jægerspris Nord with a daily meeting place in Jægerspris. At the same time, you will become part of the therapy function across Døgnplejen Frederikssund, where therapists work closely together across the organization.
On the west side you will collaborate particularly with the therapy colleagues in Jægerspris Syd and Skibby , and you will jointly support the multidisciplinary teams across the areas. You will also be part of a unified therapy community with colleagues from Slangerup, Frederikssund North, Frederikssund Middle and Frederikssund South .
We place high priority on professional development and therefore meet across east and west for professional sparring meetings, where we share knowledge, develop practice and strengthen the rehabilitative efforts across Døgnplejen. Your primary area of work will be on the west side, but you will also have a strong professional network and sparring community with the therapists throughout Døgnplejen Frederikssund.
