Carelink Pleje seeks an engaged and professionally strong therapist who wishes to work holistically and be an active part of our holistic care initiative.

About the position

As a therapist in holistic care, you will be part of a multidisciplinary team where we work closely together to create coherence, safety and quality of life for the individual citizen. You will be working out in the citizens' own homes, where you assess, guide and support based on the citizen's resources and goals. The tasks range from rehabilitative training and ADL assessment to assistive device adaptation and collaboration with both care and nursing staff. As well as general daily care tasks

Your tasks

• Assessment and evaluation of the citizen's functional ability
• Development and follow-up of individual training programs
• Rehabilitation and maintenance training
• Guidance and sparring with care staff
• Interdisciplinary collaboration with colleagues and relatives
• Documentation and quality assurance
• Provision of personal care following assessments
• Guidance in the use of assistive devices and transfers
• Preparation of transfer descriptions
• Student work and guidance during their training periods
• Participation in weekly meetings with visitation and teams

We are looking for you who

• Is a licensed occupational therapist or physiotherapist
• Has an interest in rehabilitation and holistic care
• Has knowledge of care and can perform proper personal care
• Thrives in interdisciplinary collaboration
• Works independently and in a structured way
• Has good communication skills and an appreciative approach
• Has a driving license
