Visitation Officer - Gentofte Municipality

Do you want to help create good and coherent solutions for the elderly and citizens with disabilities?

We are seeking an engaged colleague who can translate prevention, professionalism, dialogue and legislation into good citizen pathways that create value in the daily lives of citizens.

With us you will be part of the entire Authority and will daily sit in a team of 15 health professionals caseworkers. In the team we share knowledge and brainstorm about both complex cases and daily task solving, just as we work closely with the entire Care and Health.

We work purposefully with guidance and prevention, we refer to personal care, practical help and rehabilitative interventions under the Social Services Act and the Older Persons Act, as well as to elderly- and disability-friendly housing and care homes.

Our approach to citizens is based on citizen pathways, prevention, rehabilitation, trust and self-determination. Therefore we seek a colleague who thrives in an authority role in motion, takes initiative for cross-disciplinary collaboration and contributes to coherent solutions for citizens.

Your tasks

You will among other things

• Identify citizens' functional ability, resources and need for support through conversations, home visits and cross-disciplinary dialogue
• Refer to interventions under the Social Services Act and the Older Persons Act, with a preventive focus
• Process applications for elderly- and disability-friendly housing as well as care homes
• Provide telephone guidance to citizens, relatives and partners
• Coordinate cross-disciplinary and complex citizen pathways, including participating in cross-disciplinary meetings
• Make and document official determinations

We imagine that you

• Are a trained nurse, physiotherapist or occupational therapist
• Ideally have experience with referrals or authority work
• Work in a holistic manner and can create coherence across disciplines
• Communicate clearly and build trustful relationships, preferably based on the method Strengthened Citizen Contact
• Can balance the citizen's wishes, professional assessments and the framework of the legislation
• Thrive in a daily life with both operations, development and change

The position is offered from 01 December 2026, and as a rule 37 hours per week

Salary and employment terms will be according to the applicable collective agreement and the New Pay principles, and will be disclosed before any possible interview.
