As some of our current nurses are going on training/competence development, the home care in Nivå is now seeking 2 engaged nurses who value working closely with the citizen – both in the nursing clinic and in the citizen's own home.

Here you will have the opportunity to work with diverse tasks and think innovatively in your task solutions in collaboration with different professional groups.

One position is 37 hours per week, day shifts with a weekend shift every 6th weekend. The other is 20-25 hours per week, day shifts with a weekend shift every 6th weekend.

Who are we?

Nivågruppen consists of 2 teams with social and health care helpers, care employees, house assistants, therapists, a pharmaconomist, social and health care assistants as well as nurses.

We work from the citizen's perspective and are well on our way to implementing the Elderly Act.

We place emphasis on multidisciplinary collaboration. We have weekly multidisciplinary meetings in the two teams as well as daily coordination and triage meetings, where we jointly strengthen the focus on rehabilitation and early detection.

In Fredensborg Municipality, nurses are self-scheduling.

Typical tasks in the Nivå group

• Chair multidisciplinary and triage meetings
• Provide nursing, care and nursing to citizens with complex nursing tasks
• Quality assure care trajectories for citizens, e.g., in acute illness, palliative care, wound care, pain management, procedures during admissions and discharges, as well as particularly complex pathways
• Advise and educate colleagues on nursing-related challenges, both current and in areas where you have particular expertise

We are looking for a nurse who finds it exciting to

• Provide nursing care to elderly citizens in the primary sector
• Plan, perform and evaluate high-quality nursing care in collaboration with other professional groups
• Lead with good documentation practices and support other professional groups' competencies in documentation and triage
• Collaborate with many different colleagues on both planned and more acute tasks
• Work with documentation in Nexus
