Citizen-Centric Health is a new clinical department established on 1 January 2026 as part of the Health Reform. From 1 January 2027 we will bring together the responsibility for acute nursing, health and care facilities and patient-facing prevention.

Our task is clear: To bring advanced nursing, treatment and assessment close to the citizen’s daily life, and to create equality in health and coherence across hospital, municipality and general practice.

In the transition, The Regional Acute Nursing plays a central role, and you will have the opportunity to

• work closely with the citizen, the municipality and general practice – and with OUH backing
• gain real clinical autonomy and influence on decisions
• be involved in shaping workflows, culture and professional direction from day one.

You will become part of a unified unit, across the Funen municipalities, based at the Health and Care Facilities in Søndersø and reporting to the management of Acute Nursing.

Your role

You will be a central clinical actor in the close health system and will among other things need to:

• handle acute and subacute tasks in the citizen’s own home
• conduct independent clinical assessments and initiate treatment
• work closely with physicians on diagnostics and treatment plans
• help prevent unnecessary hospitalisations and support earlier discharges
• ensure coherent patient pathways across sectors
• be a professional sparring partner for colleagues – both by phone and in person

We are looking for you who

• have solid experience from e.g. municipal acute nursing, emergency department, intensive care, anaesthesia or home nursing (i.e. you do not necessarily need the specialist training)
• are comfortable with independent clinical decisions – even when things move fast
• are motivated to help build something new in close collaboration with colleagues and management
• have strong clinical observation and assessment skills
• are able to build trust with citizens and partners

Employment terms and application

This is permanent positions at 37 hours per week in mixed shifts.

The Region is obliged to inform you of the starting salary for the position. You will be informed of base salary, centrally fixed allowances and pension in the Health Collective – Agreement 32.17.1.

In addition to the starting salary, the pay may include negotiated allowances depending on the position, your experience and qualifications.

A driver's license category B is required
